1 表題 : 固定による動的制動効果の再現性の検討 著者名 : 大木琢也, 行田直人, 神内伸晃, 岡本武昌 明治国際医療大学保健医療学部柔道整復学科 要旨 目的 固定材料を用いた研究では, 固定材料の違いによる固定力の評価や, テーピングの貼付による足部の制動効果についての検討がなされている. しかし, テーピングなどの教本には, 曖昧な表現が多く, テーピングを施行する者の定義は, 医療系有資格者 や 充分な経験を積んだ者 が施行しており, 定量的に定まっていない. そこで本研究の目的は, テーピング施行の各施術 各施術者間での関節制限の再現性の手法を検討することした. 方法 テーピング施行される対象 ( 以下 ; 対象者 ) は健常成人男性 1 名とした. 施術者は有資格者 ( 以下 ; 資格者 ) と資格を取得していない者 ( 非資格者 ) を選出した. 各施術者には, 対象者の右足関節に内反捻挫予防を目的としたテーピングを施行させ, 対象者に運動課題をさせた. テーピングは施術者に各 10 回ずつ施行させ, 施行ごとに運動課題を行わせ計測を行った. また, テーピング施行 1 回目,5 回目と 10 回目の関節制動について検討を行った. 計測は, デジタルビデオカメラを 4 台で運動課題を撮影し, この映像から映像編集ソフトや 3 次元動作解析システムにおいて関節可動域を算出した. 結果 考察 結果は, 足関節底屈や内反運動で, 非資格者は資格者と比較すると, 足関節角度の制限が小さい傾向であった. また, 非資格者は, 複数回テーピング施行により関節の制限にばらつきが生じ, 資格者では一定の結果を得られた. このことから, テーピング施行の再現性を検討する手法として, 有用であることが考えられた. Abstract Purpose In the research that uses the locking material, the discussion about the braking effect of the foot by the assessment of immobilizing 1

However, "a medical care system qualified person" and "enough experienced people" enforce it, and, in doctrine such as the taping, the definition of the person that a lot of vague expression enforces taping is not decided quantitatively. Then, it aimed to compare the enforcements of healer's taping, and to discuss reproducibility among each medical treatment and each healer in the present study. Method The object was assumed to be one normal adult man. The healer elected the person (As follows; non-licentiate) who was not acquiring the qualification with good and lawful men (As follows; licentiate). To each medical treatment person, I let the right foot joint of the target person enforce taping for the purpose of the varus sprain prevention and let a person of object do an exercise problem. The taping let a surgical operation person take effect by for each ten times and I let you perform an exercise problem every enforcement and measured it. In addition, I examined joint braking of the taping enforcement first, the fifth and the tenth. I photographed an exercise problem in four in digital video camera, and the measurement calculated joint excursion in picture editing software and a three-dimensional movement analysis system from this picture. Result and Discussion After the enforcement of the taping, both licentiates and non-licentiate got the restriction seen in the angle of the ankle joint. In addition, as for the scholar of non-qualification, unevenness occurred for the limitation of the joint because of the multiple taping enforcement, and a constant result was got in the people of qualification. From this, as technique to examine plasticity of the taping enforcement, it was thought that it was useful. 2
3 キーワード : 固定, 再現性, 動作解析 はじめに急性外傷などに対し施術 後療法に関節固定を目的に使用する固定材料には, テーピング, 包帯, 金属副子などがある. テーピングなどの技術書には, 貼付走行のみが述べられている事が多い. しかし, 実践では技術書に記載されている貼付走行の テープの貼り方 だけでなく, 解剖学的知識やバイオメカニクス等の知識が要求される. また, 貼付走行以外のポイントとしても, 適度な張力で引っ張る, 施行後には血管をしめすぎていないか爪の色をチェックする などの曖昧な表現が多い 1). また, 全身の様々な部位に適切なテーピングを施すためには, 多くの基本技術を組み合わせる必要があるため, 熟練した技術を要する. 従来のテーピングの研究では, 関節の制動 筋の活動量の増加などのテーピングの効果について多く検討されている 2), 3 ). しかし, 研究におけるテーピングの施行は 有資格者 や 充分な経験を積んだ者 だけが施行しており, テーピング施行時の定義が定まっておらず, テーピング施行に対して関節の制限についての再現性への検討が少ない. また, テーピング施行時における施術者の動作 ( テープの張力や組織を圧迫する度合などの感覚 ) は重要であり, テーピング貼付が身体に及ぼす影響に差が生じるものだと考えられた. そこで本研究の目的は, テーピング施行の各施術 各施術者間での関節制限の再現性の手法を検討することした. 対象および方法テーピング施行される対象 ( 以下 ; 対象者 ) は健常成人男性 1 名とした. 施術者は有資格者 ( 柔道整復師, 以下資格者 )6 名 ( 平均年齢 30.1±5.2 歳 ) と資格を取得していない者 ( 明治国際医療大学の学生, 以下非資格者 )7 名 ( 平均年齢 21.3±0.5 歳 ) の計 13 名とした. なお資格者は, 資格を習得してから 3 年以上経過している者とし, 非資格者は既にテーピングの授業を修了している本学 3 4 年生を対象とした. 各施術者には, 対象者の右足関節にテーピングを施行させ, 対象者に運動課題をさせた. テーピングは, 成書に記載されている足関節内反捻挫予防 ( 以下 ; 内反捻挫予防 ) を目的とした, アンカー, スターアップ, フィギュアエイト, ヒールロック法を施行させた. テーピングは施術者に各 10 回ずつ施行させ, 施行ごとに運動課題を行わせ計測を行った. また, テーピング施行 1 回目,5 回目と 10 回 3
4 目の関節制動について検討を行った. 使用テープは幅 38 mmの非伸縮テープ ( 日東メディカル社製 ;CB38) を使用した. またテーピングを施行する際, 被験者の皮膚を保護するために幅 70mm の皮膚保護用ラップ ( 日東メディカル社製 ;UW70) を使用した. 運動課題は, 股関節 膝関節伸展位, 足関節 90 ( 背臥位 ) の肢位から足関節が最大底屈位 ( 図 1) になるように指示した. 次に, 股関節 膝関節伸展位, 足関節最大底屈位から足関節が最大内反位 ( 図 2) になるように指示した. 図 1 足関節底屈運動と角度算出法 図 2 足関節内反運動と角度算出法 また運動速度は, メトロノームの 60bpm に合わせて 1 拍で 1 回 (2 拍で 1 往復 ) の運動とした. 計測は, デジタルビデオカメラ (CASIO 社製 ; EX-F1)4 台で運動課題を撮影し, 映像はパソコンに取り込んだ. その後, 映像編集ソフト (Canopus 社製 ;EDIUS NEO) において映像を編集し,3 次元動作解析システム (DKH 社製 ;Frame-DIASⅣ ) において画像上の座標値から 3 次元 DLT 法を用いて被験者のマーキングポイントの実座標 ( 関節角度 ) を算出した. DLT 法 (Direct Linear Transformation method) とは, 複数のカメラを用いて 3 次元位置座標を推定する計測方法である 4 ). カメラから得られる情報は 2 次元情報であり, 1 台のカメラから直接 3 次元座標を推定できない為, 一般には複数のカメラが必要となる 5). 関節位置の同定のために使用した反射マーカーの貼付部位は, 脛骨粗面 下腿中央部 足関節中央部 第 2 中足骨頭とした ( 図 3). また, 関節角度を計測する際に, 下腿中央部から関節中心のなす線と, 関節中心から第 2 中足骨頭のなす線を結んだ線から足関節底屈角度と内反角度を算出した ( 図 1,2). 各運動の方向は, 前額面を X( 左右 ) 方向, 矢状面を Y( 前後 ) 方向, 水平面を Z( 上下 ) 方向とし ( 図 4), 結果を YZ 面 ( 足関節底屈 ) と XY 面 ( 足関節内反 ) で検討した. また, 右足関節が背屈 (Z 軸 ), 外反 (X 軸 ), 回外 (Y 軸 ) の運動で, 各軸に対して正の動きになるように設定した. 4
5 Z 軸 X 軸 Y 軸 図 3 反射マーカー貼付部 図 4 各運動方向 結果結果は, テーピング施行後の足関節の可動域を表しており, 横軸はテーピングを施行した回数, 縦軸は足関節可動域を表している ( 図 5,6). また図 5,6 のグラフは, 数値が低ければ足関節可動域に制限があり, 高くなれば足関節の制限が少なくなっていることを表している. 資格者や非資格者ともにテーピング施行後には足関節の角度 ( 底屈と内反 ) に制限が見られた. 図 5 各テーピング時の足関節底屈可動域図 6 各テーピング時の足関節内反可動域 足関節底屈運動では, 非資格者は資格者と比較すると, 足関節角度の制限が小さかった. また非資格者はテーピング 1 回目の施行,5 回目の施行と 10 回目の施行でテーピング施行の回数に比例して足関節可動域に制限が見られた. さらに非資格者のテーピング 1 回目の施行と比較して非資格者の 10 回目の施行では有意に足関節角度の制限が見られた. また非資格者のテーピング 1 回目の施行は, 資格者のテーピング 1 回目の施行,5 回目の施行と 10 回目の施行で有意な差が見られた 資格者は, 非資格者と比較して, 一定の足関節底屈可動域の制限が見られた. 足関節内反運動でも, 足関節底屈と同様に, 非資格者は資格者と比較すると足関節角度の制限が小さい傾向がみられ, 有意な差はみられなかった. 5
6 考察武藤は, 傷害予防を目的としたテーピングの論理は, 正常の運動範囲もしくは機能を制限することなく, 靭帯構造を外部から支持することにあり, この靭帯の支持 ( サポート ) は, 生理的もしくは正常の運動範囲を越えてしまった場合にのみ, 存在することが必要なものである. さらに実用的な意味では, テーピングは正常の運動範囲の極限に近づいたときに支持 ( サポート ) もしくは制限を強めるという形で目的を果すもので, できれば靭帯繊維が断裂する直前のポイントで制限しもしくはサポートが最大になるのが望ましいと述べている 1). このことから, 今回用いたテーピングは足関節の内反捻挫予防を目的としたテーピングであるため, 必要以上の制限がないことが理想であると考えた. 必要以上の制限とは, テーピングを施行される対象者によって異なり, 施術者は対象者の状態を見極めて, テーピングの張力や圧迫力を調節していくため, テーピングを施行する経験等が重要である. 資格者において足関節底屈運動や内反運動の両運動では, テーピング 1 回目の施行,5 回目の施行と 10 回目の施行の資格者間で, 関節の制限に有意な差は見られなかったことから, 資格者は非資格者と比較して関節制限を一定させながら施行していることだと推察された. 足関節底屈運動で非資格者はテーピング 1 回目の施行,5 回目の施行と 10 回目の施行でテーピング施行の回数に比例して足関節可動域に制限が見られた. また, 非資格者のテーピング 1 回目の施行を比較して非資格者の 10 回目の施行では有意に足関節角度の制限が見られた. これは, 複数回テーピングを施行していく上で, テーピング施行時の張力や圧迫力を, より強く調整しながら施行していることが考えられた. また, 計測環境の都合上, 施術者はテーピング施行後に対象者が運動課題を行う姿を見ているため, 非資格者は視覚的にテーピングによる制限の度合を確認し, 次にテーピングを施行する際により強い力で施行していったものだと推察された. 資格者に非資格者と比較して一定の足関節底屈可動域の制限が見られたのは, それまでに様々な対象に対してのテーピング施行を経験しており, 本研究は対象が 1 名で常に同側同関節であった為, 一定の結果が得られたものと推察された. 足関節底屈や内反運動で, 非資格者は資格者と比較すると足関節角度の制限が少ない傾向であったのは, テーピングの巻き方は熟知しているが, どのポイントで張力等をかけていいか理解しておらず, 今回施行した内反捻挫予防に対するテーピングの目的を意識してないものだと考えられた. 資 6
