1 676 Vol.30 No.4/Jul 術後管理における PCA の上手な使い方 IV-PCA と硬膜外 PCA(PCEA) の選択と適応 IV-PCA の適応 * 井上莊一郎 * 平幸輝 * 瀬尾憲正 [ 要旨 ] 経静脈的自己調節鎮痛法 (intravenous patient-controlled analgesia: IV-PCA) は, 投与経路の確立が容易で硬膜外鎮痛が適応とならない症例にも用いることができる利点がある. しかし, 硬膜外鎮痛と比較すると, 体動時の鎮痛効果が劣ること, 呼吸器合併症の頻度が高いこと, 消化管運動機能の回復が遅いことが欠点である. また, オピオイドのIV-PCAだけで鎮痛を得ようとすると, オピオイドの副作用によって術後回復が妨げられることもある. そこで,IV-PCA を用いる際には, ほかの鎮痛法と組み合わせた multimodal analgesia によって, オピオイドの弊害を回避しながら鎮痛効果を高める工夫をする方がよい. また,IV-PCA の適応とならない症例があることにも留意する必要がある. キーワード : 術後鎮痛, 経静脈的自己調節鎮痛法,IV-PCA,multimodal analgesia ( 日臨麻会誌 Vol.30 No.4, , 2010) はじめに patient-controlled analgesia PCA intravenous patient-controlled analgesia IV-PCA IV-PCA IV-PCA IV-PCA Ⅰ IV-PCA が有効な鎮痛法である根拠 IV-PCA PCA 1,

7 682 Vol.30 No.4/Jul Indications for Postoperative Intravenous Patient-Controlled Analgesia Soichiro INOUE, Kohki TAIRA, Norimasa SEO Department of Anesthesiology and Critical Care Medicine, Jichi Medical University We present the indications and contraindications of the patient-controlled analgesia(pca)system and intravenous PCA(IV-PCA). PCA is contraindicated in patients who cannot comprehend PCA, refuse to use PCA, or cannot operate the PCA device. Indication of IV-PCA depends on the type of surgery, duration of postoperative pain, and the start of oral intake. In other words, patients who can understand the PCA system and require parenteral opioids for their moderate to severe pain during the first few postoperative days are good candidates for postoperative IV-PCA, even young children. Furthermore, IV-PCA is a good choice for patients who are contraindicated for postoperative epidural analgesia such as the patient refusing, deformities of the spine, abnormal coagulation status, and perioperative aggressive anti-thrombic therapy. On the other hand, adverse events related to intravenous opioids, especially respiratory depression, must always be kept in mind. Improving postoperative analgesia and also avoiding adverse events related to opioids, multimodal analgesia consisted with opioid, nonsteroidal anti-inflammatory drug and neural blockade is rational choice for postoperative analgesia.
