2 日本視能訓練士協会誌 常をスクリーニングする精度をあげる機器として期待される Abstract Purpose We reported the refraction results obtained by Retinomax and Spot TM Vision Screener without cycloplegia in a health checkup for 3-year-old children. Subjects and Methods Subjects were 142 eyes of 71 children who received the health checkup for 3-year-old children in Fukushima city. Spherical equivalent refraction and cylindrical refraction measured by Retinomax and Spot TM Vision Screener without cycloplegia were compared. Results Both Retinomax and Spot TM Vision Screener could measure refraction for both eyes in all 71 subjects. The obtained spherical equivalents (mean ± standard deviation ; range) were ± 1.14 D ; to D by Retinomax, and ± 0.56 D ; to D by Spot TM Vision Screener. A significant difference was observed between the measurements by the two devices (P < 0.001, Wilcoxon signed rank test). In 121 (85%) and 30 (21%) eyes, Retinomax and Spot TM Vision Screener respectively measured spherical equivalent refraction as myopia. Measurements of cylindrical refraction (mean ± standard deviation ; range) were 0.54 ± 0.50 D ; 0.00 to 2.50 D by Retinomax, and 0.73 ± 0.56 D ; 0.00 to 3.00 D by Spot TM Vision Screener. A significant difference was observed between the measurements by the two devices (P < 0.001, Wilcoxon signed rank test). Conclusion As compared to Retinomax, Spot TM Vision Screener tended to measure spherical equivalent refraction as hyperopia because it could have avoided instrument myopia. We therefore expect Spot TM Vision Screener to be a device for a health checkup that can further improve the accuracy in screening refractive errors which cause amblyopia without cycloplegia. Ⅰ. 緒言 3 歳児健康診査における視覚検査 ( 以下 3 歳児健診 ) の目的は 屈折異常や斜視など視覚発達の阻害因子をもつ児を早期に発見することである 視力検査に併せて屈折検査を行うことは弱視や屈折異常の発見に有効であるため 3 歳児健診への導入が推奨されている 2) これまで 3 歳児健診の屈折検査については 検影法 手持ち式オートレフラクトメータ 両眼開放型オートレフラクトメータ 据え置き式オートレフラクトメータ についての報告があり それぞれの長所と短所が挙げられている 2)~ 10) 我々が携わっている福島市の3 歳児健診では 平成 12 年度より視能訓練士 2 名が参加し 全受診児に対してRetinomax (Righton 社製 ) による自然瞳孔での屈折検査を施行している 4), 1 Retinomax は 検査場所の制限が少なく 操作が容易で所要時間が短いという利点がある一方 内部視標を覗き込むため 器械近視が 誘発されやすく 調節介入が避けられない欠点がある 5 ),6 ) 調節力の強い幼児の屈折検査には 固視目標が遠方であることや十分な雲霧がかかる機器の使用が望ましく 検影法が最も望ましいとされているが 6) 検者の習熟を要することから すべての市町村に普及しがたい問題がある 一方 Spot TM Vision Screener(Welch Allyn 社製 ) は 網膜からの反射光を利用したフォトレフラクション法を測定原理とし 両眼同時に屈折検査が可能である Spot TM Vision Screener には On-axis フォトレフラクション法とOff-axisフォトレフラクション法の両方が搭載されている Off-axisフォトレフラクション法の基本原理は検影法であり これを写真法により簡便に利用できるようにしたものである 12) Spot TM Vision Screenerは小児の弱視スクリーニングにおける海外での有効性が報告されている 13)~16) しかし 本邦での3 歳児健診で Spot TM Vision Screenerの有用性を調査した報告はまだない 148
3 今回 福島市の3 歳児健診でRetinomax と Spot TM Vision Screener の 2 機種を用いて自然瞳孔での屈折検査を全受診児に施行したので その結果を報告する Ⅱ. 対象及び方法福島市の3 歳児健診は 3 歳 6ヵ月で実施されており 毎回 2 名の視能訓練士が参加し 全受診児に対してRetinomax による屈折検査を施行している 本研究の対象は 平成 28 年 6 月に福島市の3 歳児健診を受診した3 歳 6ヵ月の幼児 71 名 ( 男児 33 名 女児 38 名 ) の 142 眼である 3 歳児健診に参加している視能訓練士が Retinomax とSpot TM Vision Screener を用いて自然瞳孔での屈折値を測定した Righton 社製 Retinomax は 2 経線位相差測定方式 ( 検影法 ) を測定原理とし 内部視標により測定を行う装置である 測定は 明室で自然瞳孔下にて行い モニターで瞳孔と屈折値の確認をしながら 変動する場合には一定するまで測定した ( 詳細は 4) に記載 ) Welch Allyn 社製 Spot TM Vision Screener は フォトレフラクション法を測定原理とし 両眼開放で約 1mの距離からランダムな視覚的パターンと可聴音を固視目標として測定する装置である 測定は 半暗室で自然瞳孔下にて行った 2 機種の等価球面屈折値および円柱屈折値の結果を比較し Wilcoxon 符号付順位検定を用いて有意差検定を行った 等価球面屈折値が0.00Dより小さい値を近視 ±0.00Dを正視 0.00Dより大きい値を遠視と判定した 連結不能匿名化方式とし Retinomax およびSpot TM Vision Screenerの測定値と性別のみを収集した Ⅲ. 結果 Retinomax とSpot TM Vision Screenerは 71 名の全受診児において共に両眼の屈折値測定が可能であった 等価球面屈折値の分布範囲は Retinomax では-5.00~ +4.00D( 図 1A) Spot TM Vision Screener では-1.125~ +3.75D( 図 1B) であり Retinomax の方が近視側に広い分布となっていた 等価球面屈折値の平均値はRetinomax では-1.19 ±1.14D Spot TM Vision Screenerでは ±0.56D 中央値はそれぞれ-1.25D +0.25D であり 2 機種の等価球面屈折値に有意差を認めた (Wilcoxon 符号付順位検定,p<0.00 等価球面屈折値が近視に測定されたものは Retinomax では121 眼 (85%) Spot TM Vision 図 1. 等価球面屈折値の分布ヒストグラム Retinomax の結果を (A) Spot TM Vision Screener の結果を (B) に示す 149

5 図 4.Retinomax と Spot TM Vision Screener の円柱屈折値の散布図 Screenerでは30 眼 (21%) で 等価球面屈折値が正視または遠視に測定されたものは それぞれ21 眼 (15%) 112 眼 (79%) であった 142 眼中 132 眼 (93%) は Retinomax に比較してSpot TM Vision Screener の方が遠視側に測定されていた ( 図 2) 最も遠視に測定された 1 眼はRetinomax では +4.00D Spot TM Vision Screenerでは +3.75D であり 同一児の同一眼であった Spot TM Vision Screener の測定値からRetinomax の測定値を引いた値は +1.47± 1.06D( 分布範囲 ~ +4.75D) であった 円柱屈折値の分布範囲は Retinomax では 0.00 ~2.50D( 図 3A) Spot TM Vision Screener では 0.00 ~3.00D( 図 3B) であり Spot TM Vision Screener の方が広い分布となっていた 円柱屈折値の平均値はRetinomax では 0.54±0.50D Spot TM Vision Screenerでは0.73±0.56D 中央値は2 機種とも0.50D 最頻値は Retinomax では 0.25D Spot TM Vision Screener では 0.50D であり 2 機種の円柱屈折値に有意差を認め (Wilcoxon 符号付順位検定,p<0.00 Spot TM Vision Screenerの方が大きく測定される傾向があった 2 機種の円柱屈折値が一致していたものが 38 眼 (27%) であった 円柱屈折値が不一致であった104 眼 (73%) のうち 25 眼 (18%) は Retinomax で 79 眼 (56%) はSpot TM Vision Screenerで大きく測定されていた ( 図 4) 2 機種の円柱屈折値の差は 0.19±0.41D( 分布範囲 -1.50~ +0.75D) であった Ⅳ. 考按我々は福島市の3 歳児健診を受診した10,454 人を調査し Retinomax による検査可能率は 98% であると報告した 4) 一方 1~12 歳 108 名を対象としたSpot TM Vision Screenerの調査では4% が測定不能であったと報告されている 14) 今回の3 歳児健診受診児を対象とした検査可能率はRetinomax Spot TM Vision Screenerともに100% であった 従って 高い検査可能率の 151
6 日本視能訓練士協会誌 面では 2 機種とも健診に有用であるといえる 本研究では 等価球面屈折値は Spot TM Vision Screenerで Retinomax より有意差をもって遠視側に測定されていた 神田らは 3 歳児健診受診児の1,330 眼を対象に検影法による屈折検査を行い 弱主経線の屈折値は ±0.67Dであったと報告している 3) 本研究での Spot TM Vision Screener で測定された等価球面屈折値の平均値は +0.28±0.56Dであり 調節介入が少ないとされている検影法の弱主経線屈折値と同等であった 従って 3 歳児健診においてSpot TM Vision Screener は遠視の発見に有用であると考えられる Retinomax は器械を眼に近づけ 内部視標を覗き込んでもらい測定するため器械近視が混入しやすい 5) 6) 我々は 福島市の3 歳児健診で要精査とされ医療機関を受診した児を対象に 医療機関で測定された1% アトロピン点眼による調節麻痺下屈折値と健診時に Retinomax で測定した自然瞳孔下屈折値を照合し 弱視の要精査基準を検討した 17) その結果から Retinomax を3 歳児健診で用いた場合 2.00D 以上の等価球面屈折値の左右差を持つ遠視性不同視弱視の要精査基準は 等価球面屈折値の左右差が2.00D 以上あり かつ 少なくとも1 眼が遠視に測定される また 2.00D 以上の乱視による屈折異常弱視の要精査基準は 乱視が1.50D 以上に測定される という基準を定めた 17) しかし Retinomax では器械近視の影響により 自然瞳孔で遠視を検出することは困難であるため 屈折異常弱視の要因となる遠視に対する明確な基準を設けることはできなかった 17) 本研究では要精査とされ医療機関を受診した児での照合比較は行っていないが Spot TM Vision Screenerは遠視の検出に優れていることから これまでRetinomax でできなかった屈折異常弱視の要因となる遠視のスクリーニングにも基準を設けることが可能かもしれない 本研究では 2 機種の円柱屈折値の合致率は 27% と低く Spot TM Vision Screener でRetinomax より有意差をもって大きく測定されていた 学童を対象とした米国の検討では 自然瞳孔で測 定したSpot TM Vision Screenerでの円柱屈折値は 調節麻痺下で測定したRetinomax での円柱屈折値に比して 大きく測定されたとの報告がある 15) 一方で 我々が福島市の3 歳児健診で要精査とされ医療機関を受診した児を対象に行った検討では 健診時に自然瞳孔で測定した Retinomax の円柱屈折値は 精査時に調節麻痺下で測定した据置式オートレフラクトメータ (ARK700-A NIDEK 社製 ) の円柱屈折値に比して 小さく測定される傾向があった 17) これらのことから 円柱屈折値はSpot TM Vision Screenerで過大評価されるのか Retinomax で過小評価されるのかについては 今後 多数例での検討を要すると考える Spot TM Vision Screenerの他の利点としては 基準値をあらかじめ入力しておけば異常値は赤文字で画面表示されることや両眼の屈折値と眼位のスクリーニングが同時に行えること 操作が簡便で測定時間が短いことなどが挙げられる 16) Retinomax と比較してのSpot TM Vision Screenerの欠点は等価球面屈折値の測定範囲が-7.50D~ +7.50D 円柱屈折値の測定範囲が 0.00D~ +3.00Dと測定範囲の狭さだが 測定範囲外は要精査と表示されるため スクリーニングに用いるには問題ないと考える 従って Spot TM Vision Screenerは自然瞳孔での健診の精度を上げる機器として期待される 今後 3 歳児健診でSpot TM Vision Screener を用いた場合の弱視の要因となる屈折異常もしくは遠視の要精査基準についてさらに検討が必要である 参考文献 1 ) 日本眼科医会公衆衛生部 : 三歳児健康診査における眼科検診の手引. 日本の眼科 61: 別冊, ) 内海隆 : 三歳児健診の屈折検査について. 眼臨 101:22 25, ) 神田孝子, 川瀬芳克, 山口直子, 内田尚子 :3 歳児の屈折分布について. 眼臨 80: , ) 齋藤かおり, 森隆史, 根津吉史, 清野あか 152
