髄膜炎診断における jolt accentuation の検査特性 (130220 140812) (140812) 参考文献 3 4 を読んで追記 以前 jolt accentuation について勉強した時には 感度がかなり高く 除外にとても有用な身体所 見であると勉強した この時読んだ論文では感度が 100% とかなり大胆に紹介されていた Among patients with fever and headache, jolt accentuation of headache is a useful adjunctive maneuver, with a sensitivity of 100%, specificity of 54%, positive likelihood ratio of 2.2, and negative likelihood ratio of 0 for the diagnosis of meningitis 髄膜炎の診断 ( どんな所見をとればいいの?) http://rockymuku.sakura.ne.jp/sinnkeinaika/zuimakuennnosinndann.pdf この論文の根拠となった日本人の研究の原文 2) では 少数の研究であるものの 感度 97.1% 特異度 60% と記載されている いずれにしても感度が非常に高いというイメージがあるのではな いだろうか Among 34 patients with pleocytosis, 33 had jolt accentuation (sensitivity: 97.1%), while only 5 of them had neck stiffness or Kernig's sign. Among 20 patients without pleocytosis, 12 had no jolt accentuation (specificity: 60%). We found jolt accentuation to be the most sensitive sign of CSF pleocytosis. 今回読んだ論文 1) は これらの論文とはまったく逆の結果を示した論文 実際の臨床の感 覚ともずれているような気もするし どう読んだらいいのかちょっと困ってしまう No physical sign of meningeal irritation could accurately distinguish those with and without meningitis: nuchal rigidity (LR+ 1.33 (0.89, 1.98) and LR- 0.86 (0.70, 1.06)), head jolt accentuation of headache (LR+ 5.52 (0.67, 44.9) and LR- 0.95(0.89, 1.00)), Kernig's sign (LR+ 1.84 (0.77, 4.35) and LR- 0.93(0.84, 1.03)) and Brudzinski's sign (LR+ 1.69 (0.65, 4.37) and LR- 0.95 (0.87, 1.04)).
( 参考文献 1 より引用 ) 上記の 2 2 の表を見てみると jolt sign が陽性の患者が非常に少ないような気がする 全体の 4% 弱の患者しか頭痛の増悪を認めないということだ 頭を振らせると痛みの増悪を訴える患者は多いような気がするし どちらかと言えば偽陽性が多いと感じていたが ( 少なくともこの試験の対象になっているような 熱や頭痛 精神状態の変化などを認めるような患者では ) we prospectively enrolled all consecutive patients with AES (fever, headache, and altered mental status, with or without seizures or focal neurological deficit) in whom treating physicians had ordered CSF examination. 感度が 6.06% 特異度が 98.9% ということだから 以前に発表されている論文の内容とはかなり かけ離れている 文献 3 によると感度は 63.9% 特異度 43.2% とされている これをもとに尤度比を計算すると 陽性 尤度比は 1.13 陰性尤度比は 0.84 と計算できる The sensitivity and specificity of jolt accentuation of headache were 63.9% (95% confidence interval, 51.9%-76.0%) and 43.2% (34.7%-51.6%), respectively.

( 参考文献 4 より引用 ) ( 参考文献 4 より引用 ) 今後 髄膜炎を疑うような患者で jolt accentuation の結果が陽性 陰性どちらだったとしても 迷う度合いが大きくなってしまう気がする jolt 陰性でも安易に否定せず 重症度や状況によっては次のステップに早めに進むということも必要かもしれない まれな疾患だけれども 重要な疾患なので しばらくは保守的に対応していこうと思う 今後発表される論文にも注目していきたいと思う ( 今のところ この所見が陰性だから疾患は無いだろう と 単一の所見で決めてしまうことはやっぱり危険というメッセージとして受け取っておきたい ) 経験が豊富な神経内科や救急の先生の実感のようなものはどうなのだろうか
