If you apply to the Branch Office or the District Immigration Office, please fill out the name of Regional Office which have jurisdiction over the Branch Office or the District Immigration Office. 添付する写真の裏面には, 必ず氏名を記入してください Please make sure to write your name on the back side of your photo. 申請人が有する国籍を記入してください 台湾, 香港, マカオの場合は台湾, 中国 ( 香港 ), 中国 ( マカオ ) と記入してください Please write your country name. If you have Taiwanese or Hong Kong Nationality,please write "Taiwan","China (Hong Kong)" or "China (Macau)". 英字氏名を姓, 名の順に記入してください Please write your family name in English first, and write your given name in English. 13 番 : 卒業までに必要な年月数を記入してください Please write as number of years and months until graduation.
