1 308 Vol.32 No.3/May 2012 日本臨床麻酔学会第 30 回大会シンポジウム日臨麻会誌 Vol.32 No.3, , 2012 術前絶食期間を考え直そう ~ Let's Reconsider Preoperative Fasting Period ~ 術前経口補水療法は安全な術前体液管理方法である C 呼気ガス診断を応用した胃排出能検査法を用いた検討から * 谷口英喜 [ 要旨 ] 摂取飲料の種類による胃排出速度の違いに着目し, ミネラルウォーター (mineral water: MW), 経口補水液 (oral rehydration solution:ors) および高濃度炭水化物含有飲料 (hyper concentric carbohydrate drink:hcho) に関して C 呼気ガス診断を応用した胃排出能検査法による検討を行った. その結果,ORS は MW に比べて, 呼気中 CO 2 / 12 CO 2 の基準値 ( 投与前値 ) に対する変化量比がピークとなるまでの時間 Tmax(maximum drug concentration time) 値が速い傾向にあった (P=0.05). さらに,ORS は HCHO に比べて,Tmax 値が有意に速かった (P=0.04). よって, 胃排出速度の点から経口補水療法 (oral rehydration therapy:ort) は, 患者および麻酔科医にとってより安全な術前の経口的な補水方法と考える. キーワード : C 呼気ガス診断, 経口補水液, 経口補水療法, 術前経口補水療法, 胃排出速度はじめに oral rehydration therapy ORT 1 ORT WHO ORT oral rehydration solution ORS 2 ERAS enhanced recovery after surgery protocol hyper concentric carbohydrate drink HCHO 3, 4 mineral water MW clear fluids clear fluids oral nutritional supplement ONS 表

9 316 Vol.32 No.3/May 2012 Preoperative Oral Rehydration Therapy is a Safe Preoperative Fluid and Electrolyte Management Method:Gastric Emptying Rate Assessed by C Acetate Breath Test Method Hideki TANIGUCHI Department of Nutrition and Dietetics, Kanagawa University of Human Services Gastric emptying rate is an interesting value which varies with the kind of consumed liquid. We investigated the gastric emptying rate of various drinks by C acetate breath testing. Gastric emptying was measured for mineral water(mw), oral rehydration solution(ors), and high concentration carbohydrate drink(hcho). The results showed that the maximum concentration time(tmax), which is the time to reach a peak value of CO 2 / 12 CO 2 in the expired air relative to the baseline(value before consuming a drink), tended to be fast in the ORS as compared with the MW(P=0.05). Also, the Tmax was significantly faster in the ORS as compared with HCHO(P=0.04). Based on the results of the gastric emptying rate, oral rehydration therapy(ort)is considered to be a safe oral rehydration method for patients, as well as for anesthesiologists.
