1 臨床血液第 75 回日本血液学会学術集会リンパ系腫瘍 :MM EL-36 ガイドライン ( 標準治療 ) 多発性骨髄腫ガイドライン畑裕之 Key words : Multiple myeloma, Guideline, Treatment はじめに多発性骨髄腫に対する多くの新薬の登場は, 飛躍的な治療選択肢の増加を実現したが, 適切な治療選択に悩む状況をもたらしているこの状況を整理すべく,National Comprehensive Cancer Network (NCCN) のガイドライン 1),British Committee for Standards in Haematology (BCSH) and UK Myeloma Forum のガイドライン 2, 3),Mayo Clinic のガイドライン 4), 日本血液学会造血器腫瘍ガイドライン (2013 年出版予定 ) などの骨髄腫ガイドラインが刊行されている各ガイドラインの実際には大きな違いがないが, ガイドラインごとに特色がある NCCN ガイドラインは簡便かつ具体的であり,BCSH/UKMF のガイドラインは, 病状を細分化し, きめ細かい対応が考慮されている Mayo Clinic のガイドラインは,3 つのリスク層別化を最初に行い, リスクに応じた対応を示しているなかでも日本血液学会ガイドラインは日本の現状に即したものであり,clinical questions とその回答を記載することで, ガイドラインの根拠を明確に提示しており, 読者に理解されやすい本稿では, 日本血液学会のガイドラインを引用し, その骨子を概説するなお, 各項目の詳しい参考文献は, ガイドライン本文に記載されている日本血液学会造血器腫瘍ガイドラインでは,National Comprehensive Cancer Network (NCCN) の推奨基準を採用している以下に NCCN の推奨度を示すが, 本文中にも適宜記載した推奨カテゴリー 1 高いレベルのエビデンス ( 大規模無作為化比較試験や熊本大学生命科学研究部生体情報解析学メタアナリシス ) に基づき, パネルメンバーによるコンセンサスの一致がある推奨カテゴリー 2A 低いレベルのエビデンスしかないが, パネルメンバーによるコンセンサスの一致がある推奨カテゴリー 2B 低いレベルのエビデンスしかなく, パネルメンバーによるコンセンサスも一致がない ( しかし, 大きな反対ではない ) 推奨カテゴリー 3 パネルメンバーのほとんどが反対している 1. 診断基準と奏功判定基準ガイドラインを使用するにあたり, 骨髄腫の診断基準, 治療奏功および増悪基準を正確に適用する必要がある世界的には International Myeloma Working Group (IMWG) の基準が広く用いられ 5), 日本血液学会もこの基準を用いている (Table 1 3) 2. 治療の流れ a) 初回治療症候性骨髄腫のみを治療対象とし, 無症候性骨髄腫は無治療で経過観察とする次に治療対象症例を幹細胞移植併用メルファラン大量量療法の適応例か否かに区別する移植適応例: 日本血液学会造血器腫瘍ガイドラインでは,65 歳を境に自家末梢血幹細胞移植適応を決定し, 移植適応例は導入療法後に幹細胞採取を行い, メルファラン大量療法を行う ( 推奨グレード, カテゴリー 1,Fig. 1) その後の地固め, 維持療法は, ガイドラインでの推奨は得られていないその理由は, サリドマイドによる地固め, 維 (1850)304

Response Category scr stringent complete response CR complete response VGPR very good partial response PR partial response SD stable disease not recommended for use as an indicator of response; stability of disease is best described by providing the time to progression estimates Response Criteria CR as defined below plus: Normal free light chain FLC ratio and absence of clonal cells in bone marrow by immunohistochemistry or immunofluorescence Negative immunofi ation on the serum and urine and disappearance of any soft tissue plasmacytomas and 5 plasma cells in bone marrow Serum and urine M-protein detectable by immunofi ation but not on electrophoresis or 90 reduction in serum M-protein plus urine M-protein level 100 mg per 24 h 50 reduction of serum M-protein and reduction in 24 h urinary M-protein by 90 or to 200 mg per 24h If the serum and urine M-protein are unmeasurable, a 50 decrease in the difference between involved and uninvolved FLC levels is required in place of the M-protein criteria If serum and urine M-protein are unmeasurable, and serum free light assay is also unmeasurable, 50 reduction in plasma cells is required in place of M-protein, provided baseline bone marrow plasma cell percentage was 30 In addition to the above listed criteria, if present at baseline, a 50 reduction in the size of soft tissue plasmacytomas is also required Not meeting criteria for CR, VGPR, PR, or progressive disease 持療法は, 全生存期間の延長を保証するとの報告もあるが否定的な報告もあり, まだ結論は得られておらず, さらに, サリドマイドによる維持期間中の末梢神経障害の増悪が懸念されるためであるレナリドミドによる維持療法では,PFS,OS の延長が報告されているものの 2 次発がんの増加が報告されているため, 慎重な対応が求められているこれらの理由で, 維持療法は現在のところガイドラインでの推奨を得ていない ( 推奨グレード, カテゴリー 2B) NCCN では, 未治療例のなかで孤発性形質細胞腫を独立して取り扱い, 放射線照射を推奨しているまた, NCCN では移植後に維持療法,2 回目の自家移植 ( タンデム移植 ), 経過観察の 3 つの選択肢が記載されているタンデム移植の有用性は新薬登場以前のフランスの臨床試験で証明されているが 6, 7), 現在ではその有用性は低下していると日本血液学会ガイドラインに記載されてい 305(1851)

The absolute increase must be 10 mg/dl Bone marrow plasma cell percentage: the absolute must be 10 Definite development of new bone lesions or soft tissue plasmacytomas or definite increase in the size of e isting bone lesions or soft tissue plasmacytomas Development of hypercalcemia corrected serum calcium 11.5 mg/dl or 2.65 mmol/l that can be attributed solely to the plasma cell proliferative disorder Clinical relapse requires one or more of: Direct indicators of increasing disease and/or end organ dysfunction CRAB features. It is not used in calculation of time to progression or progression-free survival but is listed here as something that can be reported optionally or for use in clinical practice Development of new soft tissue plasmacytomas or bone lesions Definite increase in the size of e isting plasmacytomas or bone lesions. A definite increase is defined as a 50 and at least 1 cm increase as measured serially by the sum of the products of the cross-diameters of the measurable lesion Hypercalcemia 11.5 mg/dl 2.65 mmol/l Decrease in hemogloblin of 2 g/dl 1.25 mmol/l Rise in serum creatinine by 2 mg/dl or more 177 mol/l Any one or more of the following: Reappearance of serum or urine M-protein by immunofi ation or electrophoresis Development of 5 plasma cells in the bone marrow Appearance of any other sign of progression i.e., new plasmacytoma, lytic bone lesion, or hypercalcemia Fig. 1 Flow of therapeutic decision proposal by JSH guideline for transplant candidate. 4 臨床血液 54:10 る同種移植は日本血液学会,NCCN どちらのガイドラインでも推奨されていないその理由は, 自家移植に続く同種移植 (auto-mini allo) はタンデム自家移植と全生存率で差がなく, ミニ移植の単独療法は標準治療となるエビデンスがないためとされている ( 推奨グレード, カテゴリー 3) 移植非適応例: 導入療法を施行し, 奏功すれば無治療経過観察または臨床試験による維持療法を行うことが推奨されている (Fig.2) b) 再発難治例自家移植後 6 ヶ月以上経過しての再発であれば, 初回治療に対する感受性が残存する可能性があり, 同じ内容で再治療することも選択される移植が可能な例では, 救援療法, 自家末梢血幹細胞移植併用メルファラン大量療法, 同種移植から選択し, 移植不能例では救援療法を行う施行しうる臨床試験が選択可能であれば考慮するどちらの場合も奏功しない場合は, 最善の補助療法を行う (Fig.3) 3. 治療薬の選択米国ではサリドマイド, レナリドミドが初回治療として認可されており, ポマリドマイドについては, レナリドミドおよびボルテゾミブを含め, 少なくとも 2 種類以上の前治療を受け, 最後の治療から 60 日以前に疾患進行を示している場合という制限付きではあるが認可されているこのため当然,NCCN のガイドラインの方が, 日本血液学会造血器腫瘍ガイドラインよりも選択肢が多い日本では, 状況は異なるが, 今後米国と同様の選択が可能となると考えられるため, 参考として NCCN ガイドラインでの使用薬剤についても一部概説する日本血液学会造血器腫瘍ガイドラインで取り上げられているレジメンを Table 4 にまとめる a) 初回治療移植適応例の初回治療: 日本血液学会造血器腫瘍ガイドラインでは保険適応の治療として BD,BAD 療法が推奨されている ( 推奨グレード, カテゴリー 1) Ld,BTD 療法も推奨されているが, これら IMiDs を含む治療は, 今のところ日本では初回治療としては保険適応がないことに注意すべきである旧来の VAD 療法, デキサメタゾン大量療法は記載はあるものの推奨療法ではない TD,TAD,BLd は新薬を使用するもののエビデンスが乏しいうえに保険適応がなく, 推奨療法以外の選択として記載されている移植非適応例の初回治療: MP 療法と比較した臨床試験で優位性が示された MPB,MPT 療法が推奨されているが ( 推奨グレード, カテゴリー 1),IMiDs を含む治療は初回治療としては保険承認されていないため, 実際には MPB 療法のみが推奨治療となる新薬を用いるものの MP 療法との比較が不十分な Bd,Td,Ld,MPL,MPTB,CTd 療法の記載はあるものの, 推奨ではない b) 再発, 治療抵抗性の場合再発, 治療抵抗例に新規薬剤をデキサメタゾンと併用することは, デキサメタゾン単剤よりも有効であり ( 推奨グレード, カテゴリー 1), さらに新規薬剤と他の抗腫瘍薬と併用は (BOR/PLD,BOR/DEX/CPA,THAL/ DEX/PLD,LEN/DXR/DEX,LEN/CPA,DEX など ), より高い奏功率が得られることも記載されているが ( 推奨グレード, カテゴリー 2A), 日本では原則として施設 Fig. 2 Flow of therapeutic decision proposal by JSH guideline for non-transplant candidate.abbreviations are listed in table 4. Abbreviations: MHLW: Ministry of Health, Labor and Welfare of Japan, B: bortezomib, D DE : de amethasone, A: adriamycin, L: lenalidomide, T: thalidomide, HDD: high dose de amethasone, d: low dose de amethasone, CPA: cyclophosphamide, PLD, pegylated adriamycin, D R: do orubicin IRB の許可と被験者から文書同意を得た上で施行するように記載されているその理由は, 新薬とその他の抗腫瘍薬の併用は毒性が高くなること, これら併用療法が日本で臨床試験が行われておらず, 日本人への毒性が不明なためである日本で保険適応である治療として,BOR,BD,LD,T, TD,high dose CPA が記載されている移植が可能な例では, 自家移植も推奨されている 8) ( 推奨グレード, カテゴリー 2B) 4. 支持療法骨髄腫に伴う骨病変, 貧血, 感染症に対する指示療法は, 日常診療で広く行われているが, 日本血液学会造血器腫瘍ガイドラインでは, 以下の使用を推奨している a) 骨病変を有する症例へのビスフォスフォネート投与 ( 推奨グレード, カテゴリー 1) ただし, 口腔内ケアを行い顎骨壊死の予防に努めること ( 推奨グレード, カテゴリー 2A), 低カルシウム血症の発症に注意すべきであると記載されている無症候性骨髄腫に対する使用報告も散見されるが,NCCN では臨床試験において施行すべきとの立場をとっている b) ボルテゾミブ投与中のアシクロビル内服による帯状 9, 10) 疱疹予防 ( 推奨グレード, カテゴリー 2A) c) サリドマイド, レナリドミド使用中の低容量アスピ 11) リンによる深部静脈血栓症予防 ( 推奨グレード, カテゴリー 2A) ほかに NCCN では, 貧血時のエリスロポイエチン, 疼痛緩和, 骨折予防を目的とした骨病変への低容量放射線照射, 肺炎球菌ワクチンによる感染予防なども記載がある ( 推奨グレード, カテゴリー 2A) おわりにガイドラインは, 特定の条件下での臨床試験成績の平均的なエビデンスを示したものにすぎない実臨床では, ガイドラインを参考にしつつ個々の症例の状況に応じて総合的に治療を選択する必要があるまた, ガイドラインは, 固定的なものではなく適宜改訂されていく性質のものであるため, 常に最新の情報を参照する必要がある骨髄腫領域には, まだ解決されない疑問が数多くあり, これら疑問が解決されガイドラインに記載される様な進歩が求められている著者の COI(conflicts of interest) 開示 : 本論文発表内容に関連して特に申告なし文 1)Anderson KC, Alsina M, Bensinger W, et al.multiple myeloma.j Natl Compr Canc Netw.2011; 9: )Bird JM, Owen RG, D Sa S, et al.guidelines for the diagnosis and management of multiple myeloma 2011.Br J Haematol. 献 (1854)308
