2 日本視能訓練士協会誌 Abstract Purpose To investigate contrast sensitivity and hue discrimination ability of individuals wearing the LUTINA (Tokai Optical, Aichi, Japan) shielding lens, which was developed for healthy individuals, compared with those wearing another type of shielding lens or no lens. Methods Ten individuals with no visual function abnormalities were evaluated. The wearing conditions were as follows: 1) wearing no lens; 2) wearing a LUTINA lens; and 3) wearing an AC lens (CCP Series, Tokai Optical). Contrast sensitivity was measured using the Pelli-Robson contrast sensitivity test, and hue discrimination ability was measured with the Farnsworth- Munsell 100 hue test. Results Decreased contrast was not observed under any of the wearing conditions. Hue discrimination ability was within the normal range under all conditions. The total deviation point for hue discrimination ability was as follows, in ascending order: no lens < LUTINA lens < AC lens. A significant difference was observed between the subjects wearing no lens and those wearing the AC lens (p<0.05). Conclusion The present results suggest that there were no issues with wearing the LUTINA lens on a daily basis, because it did not affect contrast sensitivity or hue discrimination ability. Ⅰ. 緒言高齢化社会を迎え 単に長命を願うのではなく 健康寿命や生活の質 (Quality of Life) の向上を目指す意識が高まってきている 健康補助食品やサプリメントなどに関心が向けられ 視機能に関わることも例外ではない 東海光学は 眼内に入射する短波長光が網膜において 酸化ストレスを引き起す 1) ことに注目し nmをカットするレンズ用材料 UV+420cut TM を活用した遮光レンズ LUTINA ( 視感透過率 :93%) を発売した nmの光を約 40% カットしたレンズを日常的に装用することで サプリメント を摂取するように眼を健康に保つ効果が期待できるとしている 遮光レンズは短波長光を選択的にカットする光透過性を有した光学的補助具であり グレアを軽減する効果があると考えられてきた 2) その効果は羞明の軽減のみならず視力およびコ 3 ),4 ントラスト感度の改善 ) 暗順応の補助や体 5) 内時計の調整にまでおよぶとされている また 2010 年に補装具の支給対象が見直され 疾患に関係なく給付が可能となり 羞明軽減のため広く使用されるようになった 6) 遮光レンズが光暴露から眼を保護する 7) という報告は従来からあり 今後一層遮光レンズが健常者にも広 まる可能性がある 遮光レンズを健常者が日常的に装用するためには 昼間はもとより夜間も運転ができる必要がある 運転可能な眼鏡は透過率に関して基本的要求事項があり ( 応用 JIS T7331:2006) 視感透過率 75% 以上が必要とされている 8) また 色のついている眼鏡をかけていると レジャー に使われるサングラスのイメージが強くなるため レンズのカラーも透明に近いものが望まれる LUTINA は視感透過率も高く昼間 夜間の運転も可能である レンズのカラー分類も 無色レンズ であり 見た目では遮光レンズには見えない 本研究では LUTINA は健常者が日常に装用する遮光レンズであるということからコントラスト感度と色相弁別能に着目し 実験をおこなった コントラスト感度は視感透過率に依存する 9) とされ 視感透過率が高い LUTINA 装用ではコントラストには影響がないと考えらえるが 文字視標によるコントラスト感度測定装置である Pelli-Robson Contrast Sensitivity Test を使用して 読字に問題はないかを検討した 10) また 色相弁別能については川嶋らは 遮光レンズ装用において 色相混同の程度は視感透過率よりカットオフ波長に依存するとしている LUTNA は短波長成分をカットしてい 52
3 るために色知覚に何らかの影響を及ぼす可能性があると考え実験を行った 表 1 被験者のプロフィール Ⅱ. 対象及び方法 1. 被検者眼疾患がなく矯正視力 1.2 以上を有し 石原検査表 ( 国際版 ) で異常が検出されなかった21 歳 ~22 歳 ( 平均 21.4±0.46 歳 ) の女性 10 名を被験者とした ( 表 1) 実験前に各被験者にインフォームド コンセントを行い 同意が得られた者を対象とした 測定は右眼のみで実施した 2. 装置 LUTINA の他に 同じく視感透過率が高く昼夜間運転可能な遮光レンズとしてAC( 東海光学製 CCP400: 視感透過率 89%) を使用し比較した ( 図 1) 表 2に2つのレンズの特性を示した 図 2は それぞれの分光透過曲線である 図 3は 装着なしと LUTINA 装着 AC 装着で撮影した写真での見え方のイメージである コントラスト感度の測定は Pelli-Robson Contrast Sensitivity Test(HAAG=STREIT UK 製 ) を使用して行った Pelli-Robson Contrast Sensitivity Test は 文字視標を使用し 文字のサイズは固定されており コントラスト感度 100% から 0.6 % まで 0.15log 単位で変化しており 臨床の場で簡便に測定でき信頼性も高い 11) とされている 色相弁別能の測定は Farnsworth-Munsell (Luneau 製 ) を使用し 色比較 検査用 D65 蛍光ランプ ( 東芝 D65 昼光色 演色 AAA 色温度 6500K) 下で実施した 3. 手続き被験者は 遮光レンズ装用なし ( 以下 NONE とする ) LUTINA( 以下 LUTINA とする ) ACレンズ装用 ( 以下 ACとする ) の3 条件で実験を行った コントラスト感度は白昼光を使用した視力検査室 ( 約 1,000lux) において Pelli-Robson Contrast Sensitivity Testを検査距離 1m チャートの中心が被験者の視線の高さに合うように提示し 輝度は専用の輝度計を用いて測定し一定輝度を保つようにした 色相弁別能は暗室内の 図 1 LUTINA( 左 ) と AC( 右 ) 表 2 レンズの特性 図 2 LUTINA と AC の分光透過曲線 ( 東海光学カタログより改変 ) 53
4 日本視能訓練士協会誌 Ⅲ. 結果 図 3a 図 3b 図 4にPelli-Robson Contrast Sensitivity Test における各装用条件でのコントラスト感度の平均を示した コントラスト感度の平均値は LUTINAが一番高く ついでNONE ACとなった 各装用条件でのコントラスト感度について一元配置分散分析の結果 有意な差はなかった (F(2, 8)= 2.053, n.s) F-M 100hue testにおいては すべての条件で正常範囲内であり わずかなエラーを認めるも共通の傾向は認められず 色相混同は起こっていなかった 図 5にNONE LUTINA AC 各装用条件での総偏差点の平均点を示した 総偏差点はNONE LUTINA ACの順に高くなった 一元配置分散分析の結果 有意差が認められた (F(2, 8)= 5.272, p<0.05) Bonferroni 法による多重比較の結果 NONEとACにおいて有意な差が認められた (p<0.05) NONEとLUTINA LUTINAとAC 条件の間には有意な差は認められなかった 被験者の検査後の感想は LUTINAは装用の有無により 見え方は変わらなかったが AC は 黄 ~ 橙が見えにくい 暗い感じがする 似ている色の区別がつきにくい など判断が困難だったと訴える者が多かった Ⅳ. 考按 図 3c 図 3 各装用条件の見え方のシミュレーション (a)none (b)lutina (c)ac 色比較 検査用 D65 蛍光ランプ ( 約 1,000lux) 照明下でF-M 100 hue test で実施した レンズの装用順 検査の順は被験者間で無作為とした 本実験においてLUTINAを装用しても AC を装用しても コントラスト感度に関して影響はなかった 疾患による羞明を軽減するための遮光レンズでは 特に室外用としては 視感透過率が低い方が好まれるとされているが 12) 健常者では 視感透過率がかなり高くても羞明を抑えることができるとされている 13) 今回の実験の被験者は21~22 歳という年齢に限って行ったため 羞明を自覚する者はなかった 今後 LUTINA 装用におけるコントラスト感度に関しては 幅広い年齢層 屋内外 照明器具等の条件を変え検討する必要がある 54
