尾長谷 靖 他 1086 Table Fig. 1 lesion Chest roentogenogram in the left upper showing 1 Laboratory nodular lobe. findings on admission ml分 離 さ れ,嫌 気 性 菌 は 検 出 さ れ な か っ た.同 時 に 行 っ た 血 液 培 養 で は 菌 は 検 出 さ れ な か っ た が, 喀 疾 培 養 でS.agalactiaeが6 107cfu/ml,Klebsiella pnenmoniaeが6 104cfu/ml,Haemo- philus influenzaeが8 105cfu/ml分 (Table1).ド 離 さ れ た レ ー ン を挿 入 後 順 調 に 排 膿 し,そ れ と と も に 胸 部X線 球 数 やCRPは 上 の 陰 影 も 改 善 し,末 梢 血 白 血 改 善 を 示 し た た め6月14日 に て 用 か らS.agalactiaeに 充 分 IPM/CSとCLDM併 抗 菌 力 の あ るPiperacillin(PIPC)1日4gに し た が,1週 間 投 与 し て もCRP等 い た めAmpicillin(ABPc)1日3gに し か し,CRP再 変 更 が 陰 性 化 しな 変 更 した. 上 昇 し た た め,IPM/CSとCLDM 併 用 を 再 び 使 用 し た.7月6日 排 膿 が ほ とん どな く な っ た た め ド レ ー ン を 抜 去 し,7月1G日 数 が6,100/rnm3,CRP1.3mg/dlと ンサ 球 菌 が 多 数 み られ た.こ の た め 抗 菌 剤 と して し た.そ の 後7月13日 (IPM/CS)と MRIを た めClindamycin(CLDM)の た(Fig.3).し 併 用 療 法 を 開始 し か し,投 与 後 白 血 球 やCRPは 改 善 傾 向 を示 す もの の,胸 部X線 大 傾 向 を 認 め た た め6月3日 軽度 上肺化膿症 の増 に肺 内 と皮 下 に局 所 変 更 に腰 痛 精 査 目 的 に て腰 椎 の 撮 影 し た と こ ろT1強 調 画 像 に てL4L5の 椎 体 の 不 整 と その 間 の椎 間 板 の不 整 を認 め 同一 の 菌 の 全 身 播 種 の 結 果 の 腰 椎 化 膿 性 脊 椎 炎 と診 断 し た(Fig.2c).TFLX投 与 期 間 は化 膿 性 脊 椎 炎 を 合 併 し て い る こ と か ら長 期 に 使 用 を 余 儀 な く さ れ 麻 酔 下 に ドレ ー ン を挿 入 した.培 養 同 定 の結 果 皮 た.糖 下 の 膿 瘍 か らS.agalactiaeが HbAlc10,2%と 単 独 で5 108cfu/ ほ ぼ炎 症 は陰 性 化 し た た め 経 口 のTFLX1日300mgに グ ラ ム 陽 性 菌 に 強 い 抗 菌 力 を 有 す るImipenem 嫌 気 性 菌 の 関 与 も否 定 で き な か っ た 白血 球 尿 病 に つ い て は 入 院 時 血 糖378mg/dl, コ ン トロ ー ル 不 良 で あ っ た が,肺 感 染症 学 雑 誌 第71巻 第10号

In adults, infections by S. agalactiae are rare. We report an adult case of lung abscess and pyogenic spondylitis caused by S. agalactiae. A 51-year-old male was admitted to our hospital because of an abnormal shadow in the chest and lumbago on May 25, 1995. He was diagnosed as lung abscces from the chest roentgenogram and CT scan and the subcutaneous pus was aspirated. The pus culture was only positive for S. agalactiae. He was treated with IPM/CS 1 g/day and CLDM 1.2 g/day and the abscces was drained. MRI showed his lumbago was caused by pyogenic spondylitis. The underlying disease of this case was diabetes mellitus. He recovered from the infections with in about 10 weeks of antibiotic treatment.
