H. 43y. f O. m O. 左 急 性 化 膿 性 中 耳 炎,急 性 purul. acuta, 療 漏 は 大 量 に 流 れ 出 て 益 々 増 悪 し,約1週 本 剤1日3g,3回 第2病 総 量21gの 体 格 中 等 大 で あ る が,顔 貌 は苦 悶 状 で 元 気 が な 温 は38.5 で あ っ た 左 耳 の 外 耳 道 は 膿 性 耳 漏 が 大 量 に 簿 動 性 に 流 れ 出 て,鼓 し,特 に 後 上 部 が 腫 脹 し,下 膜 は発 赤 垂 し て い る の が み ら れ た 乳 様 突 起 の 尖 端 部 お よ び 後 部 に 圧 痛 著 明 で あ っ た レ線 で は 左 乳 様 突 起 部 に 漫 性 の 陰 影 が み ら れ(Fig.6),白 血 球 は12,500,血 沈 は1時 間70で 著 明 な亢 進 が み られ た 耳 漏 か ら培 養 検 査 を行 な っ た 治療経過 約1週 sin. 術 の 適 応 を 考 え た が,一 上 述 の よ うに 本症 は急 性化 膿 性 中耳 炎 か ら 間 で 電 撃 性 の 経 過 を と り,睡 眠 が障 害 され る ほ ど に 分 割 し て5日 応抗 間 静 注 し た と こ ろ, 日 か ら 耳 漏 が 著 し く減 少 し て き た の で,さ 日 に 受 診 し入 院 し た 扁 頭 痛 を 訴 え,体 acuta の 強 い 偏 頭 痛 が 継 続 し,手 日2gを2回 く,左 dex. The serum (diluted 10 fold) obtained at 30 minutes, one and two hours from intramuscular injection of 1g of CEC inhibited the growth of 209 P strain of Staphylococcus aureus. 3. Blood level concentrations. The peak blood level following intramuscular injection of 1g of CEC was 19.3 Đg/ml one hour after the injection and even after 6 hours the level was 2.1 Đg/ml. 4. Tissue concentrations. The concentration of CEC in the palatine tonsil one hour from intramuscular administration of 1 g of CEC was 4.3 Đg/g in the palatine tonsil, 1.7 Đg/g in the mucous membrane of sinus maxillaris and 1.7 Đg/g in the membrane of the inferior turbinate. The concentration of mucous membrane one hour after the intramuscular injection of 2g of CEC was 3.7 Đg/g. 5. CEC was administered to 27 patients with otorhinolaryngological infections. There was excellent in 20 (74.1%), good in 5 (18.5%) and no offect in one patient. Administration was discontinued in one patient who developed a eruption on the skin. 6. Side effects. As this agent induces pain at the injection site, a dilution with lidocaine was utilized. One pediatric patient with tonsillitis had a eruption following an intramuscular administration, however there were no thoer adverse effects having a clinical significance.
