1 MP 関節屈曲位ギプス固定による手指屈曲力への影響 MP 関節屈曲位ギプス固定による手指屈曲力への影響 福祉社会デザイン研究科ヒューマンデザイン専攻博士前期課程 2 年 渕ノ上真太郎 福祉社会デザイン研究科ヒューマンデザイン専攻博士前期課程 2 年 布施沙由理 福祉社会デザイン研究科ヒューマンデザイン専攻博士前期課程 1 年 岩合 昭直 要旨 日常の柔道整復の施術において手の骨折に遭遇する機会は多い 手の骨折の施術が他の部 位と異なる点は 手指は骨と腱が密に接しており 骨折部でそれらが癒着を生じやすいこと にある また 手の外傷すべてに言えることであるが MP 関節の伸展位拘縮を予防するこ とが必要である また 固定による廃用性筋萎縮を防止しなければならない 石黒ら 基節骨 中手骨骨折の保存療法において 外固定を装着した状態で早期運 動療法を行える MP 関節屈曲位固定でのギプス固定法の効果を報告し その固定法が広く 用いられている これまでに MP 関節固定角度と MP 関節伸展位拘縮との関係は研究されているが 筋萎 縮の観点から手指の屈曲力と固定角度の関係は検討されていない そこで 本研究は MP 関節固定角度と手指屈曲力との関係を求め 廃用性萎縮の予防に繋がるか否かについて検討 した 被験者として健常成人 10 名を用いた 被験者に MP 関節 70 屈曲位と 90 屈曲位の 2 つ の固定法を行った それぞれの固定にて 2 5 指のそれぞれの屈曲力を測定 簡易圧力分布 計測システム オクトセンス した 測定に当たってピーク値 RAW を屈曲力の指標とし た その結果 MP 関節 70 屈曲位と 90 屈曲位の固定法では 90 屈曲固定で有意に大きい 屈曲力を示した したがって MP 関節 90 屈曲位固定の方が MP 関節 70 屈曲固定よりも 各指の屈曲力は高いため 長期固定による手指の筋萎縮や筋力低下の予防につながる可能性 が認められた 363
2 1 序 手指の骨折はどの年齢層にもみられ 日常の柔道整復の施術において遭遇しやすい外傷で ある 手指は骨と腱が密に接しており 骨折部で骨と腱との癒着を生じやすく 骨折の施術 が他の部位と異なる 腫脹が強いときに指は伸展位をとることにより MP 関節の側副靱帯 が短縮し MP 関節伸展位拘縮を起こしやすい 施術に当たっては MP 関節の側副靱帯の 短縮による伸展位拘縮 骨折部での腱との癒着による可動域制限 回旋転位などの重篤な機 能障害を残さないようにしなければならない また 固定による廃用性筋萎縮や筋力低下を 防止しなければならない 石黒ら は 基節骨 中手骨骨折の保存療法において 外固定を装着した状態で早期 運動療法を行える MP 関節屈曲位固定でのギプス固定法 MP 関節が 屈曲位をとっ ていれば 整復の保持には十分であると示している を推奨している それ故 その固定法 が手指の骨折の施術によく用いられている このギプス固定法は ある程度の可動性が維持 された状態で骨癒合をもたらすことができ MP 関節伸展位拘縮や骨折部での腱との癒着な どを予防することができる MP 関節固定角度と MP 関節伸展位拘縮との関係は研究されているが 筋萎縮や筋力維持 の観点から手指の屈曲力と固定角度の関係は検討されていない 石黒らは MP 関節を で固定を行えば MP 関節伸展位拘縮を防ぐことができると報告している しかし MP 関節 の固定の角度と筋力の関係についての報告はされていない そこで 本研究 は MP 関節固定角度と手指屈曲力との関係を明らかにすることにより 手指の筋力の維持 や廃用性萎縮の予防に繋がるか否かについて検討することを目的とした 2 方法 1 被験者 健常成人 10 名 男性 7 名 女性 3 名 平均年齢 28.2 ± 4.97 歳 を被験者として用いた 2 ギプス固定法 被験者に MP 関節 90 屈曲位と 70 屈曲位の 2 つのギプス固定法 ナックルキャスト 石 に合わせ 固定 黒法 1 2 を用いた すなわち 中手骨軸と基節骨軸を角度計 90 と 70 を行った 図 1 2 その状態で 握り と つまみ 動作を行い第２ ３ ４ ５指の 屈曲力を測定した 3 測定方法 手指の屈曲力の測定は 簡易圧力分布計測システム オクトセンスを用いて 各指に貼り 付けたセンサーを木片に圧迫する力を求めた 図 3 4 計測器はあらかじめ校正器で校正 を行った 測定では 肘関節 90 屈曲位 前腕回外位 および手関節軽度背屈位にて 木片を握り 3 364

5 MP 関節屈曲位ギプス固定による手指屈曲力への影響 図 屈曲位固定でのつまみ屈曲力 図 8 第 2 5 指の合計したつまみ屈曲力平均値 4 考察 握り 動作において MP 関節 70 屈曲位より 90 屈曲位で固定を行った方が 屈曲力を 発揮することが認められた これは 深指屈筋 浅指屈筋 および虫様筋が第 2 5 指の屈 曲に関与し 個々の指の屈曲力より各指の合計屈曲力で差が現れると思われる したがって 深 浅指屈筋 虫様筋の屈曲力を判定するには第 2 5 指の合計屈曲力で判断５ ６ ７ ８ す ることが望ましいと思われる 図 6 手指の骨折後 手指の機能を再獲得するには 回旋変形 拘縮などを予防するだけでなく 筋力の低下予防７ も重要である それには患者に積極的且つ根気強い運動療法を継続させ る指導も必要である５ 今回の結果から MP 関節 70 屈曲位より 90 屈曲位の方が屈曲力 を発揮することが示された よって運動療法において力の入りやすい 90 屈曲位固定の方が 筋力低下や筋萎縮の予防に効果的であると考えられる つまみ 動作においては 第 2 指のみ 90 屈曲位と 70 屈曲位に大きな有意差が認められ た また 第 4 指が他の指と異なり 70 屈曲位で大きかった このような結果がみられた 要因として 前腕の肢位を指定しなかったことにより 肢位の違いが筋力に影響したものと 考えられる つまみ動作においては 今後 肢位を一定にした実験で明らかにすることが示 唆された 5 結論 MP 関節 90 度屈曲位固定は MP 関節 70 屈曲固定よりも各指の屈曲力は高いため 長期固 定による手指の筋萎縮や筋力低下の予防につながる可能性が示唆された 引用文献 1 石黒 隆 指節骨 中手骨骨折の後療法 MB Orthop

7 MP 関節屈曲位ギプス固定による手指屈曲力への影響 Effect of finger flexion force by cast immobilization on MP joint flexion FUCHINOUE, Shintaro FUSE, Sayuri IWAGO, Akinao Summary In osteopathy, the normal practitioner has opportunity to encounter many fractures of the hand. Treatment of fractures of the hand is different from those at other sites, such as the finger bones and tendons that are closely in contact. Further, there is a fendency of adhesion between the tendon at the fracture.as is true for all of the hand trauma, it is necessary to prevent the extension of the metacarpophalangeal joint contracture position. In addition, we must prevent disused muscle atrophy due to fixation. Burkhalter and Ishiguro et al, suggest that one follows: go to conservative treatment of bone fractures in the metacarpal bones-proximal phalanx. They have reported a method of fixation of cast immobilization in flexion exercise therapy that can be done while wearing an early external fixation. The Relationship between the extended position and joint contracture MP arthrodesis angle has been studied. However, the relationship between the bending force of the finger and a fixed angle from the point of view of muscle atrophy has not been considered. Therefore, this study examined the relationship between the bending force and hand angle arthrodesis MP, in a study on whether or not this leads lead to the prevention of disuse atrophy. Ten healthy adults were used as subjects. It was required to use two fixed flexions at to 70 and 90 angle on the flexion metacarpophalangeal joint on the subject. These were measured at each fixed point (using a simple pressure distribution measurement system Oct Sense)with a bending force of each of the fingers at 2-5. Bending force was used as an index of the peak value [RAW]; in measurement. As a result, in the fixation of flexion 70 and 90 the flexion showed a significantly greater bending force in flexion
