1 透析技術 透析会誌 42( ):85 90,2009 透析液中の細菌に対する各種メンブレンフィルター法の測定精度の検討 楢村井出 1 友隆 1 孝夫 佐藤 1 和弘 堀内 2 賢一 吉田 3 周理 いでクリニック 1 2 日本ポール ( 株 ) ポールライフサイエンスカンパニー応用技術研究所 3 日本ポール ( 株 ) ポールライフサイエンスカンパニーバイオメディカル事業部 キーワード : 細菌管理, メンブレンフィルター法, 簡便, 透析液 要旨 透析液の製造工程管理において, 細菌の汚染状態を把握し対策を講じることが, 透析患者への悪影響を防ぐために重要である. 細菌検出の手段として, 高感度に細菌の汚染状態を把握するためにはメンブレンフィルター法 ( 以下,MF 法 ) を用いて検査することが必要である. 今回われわれは,MF 法製品である日本ポール社製 37 mm クオリティモニターおよびマイクロファンネルを,R2A/TGE 寒天および液体培地使用下にて用いた. 標準菌 2 菌種 (Pseudomonas fluorescence, Methylobacterium extorquens) と臨床現場の透析液中から単離された 1 菌種 (Wild type) のそれぞれを透析液中に播種した試験液を用いて, 各種 MF 法における細菌の測定精度を, 細菌コロニー数の計測結果を基にして, 平板塗抹法 (R2A 寒天培地 ) と比較した. その結果, 今回用いた MF 法製品と培養方法との組み合わせの全てが, 基準となる R2A 寒天培地上のコロニー数 (Pseudomonas fluorescence(59.3 cfu/ / 枚 ), Methylobacterium extorquens(62.5 cfu/ / 枚 ),Wild type(38.6 cfu/ / 枚 )) に対して, 細菌回収率に必要な 70% 以上を上回っており, 良好な回収率を得ることが可能であった. 今回評価した MF 法製品は, 各工程の細菌管理に有効なツールとして十分に活用できるものと考えられた. When high sensitivity is required to monitor bacterial contamination, the membrane filtration(mf)method is desired. In this study, we evaluated the MF methods(37 mm Quality Monitor and MicroFunnel TM SP Filter Unit(Nihon Pall Ltd. Pall Life Science Company, Tokyo, Japan))with the culture media of R2A/TGE/ agar or broth. We compared the MF methods with the plate count methods based on bacterial colony numbers from both MF methods about the measurement accuracy of bacteria that exist in dialysis fluid using two standard microorganisms(pseudomonas fluorescence, Methylobacterium extorquens)and one wild type isolated from the dialysis fluid in our clinic. As a result, all data of the MF methods showed more than 70% bacterial recovery on the basis of each bacterial colony number for Pseudomonas fluorescence(59.3 cfu/plate), / Methylobacterium extorquens(62.5 cfu/plate)and/ Wild type(38.6 cfu/plate)on/ R2A plate agar. 2 86 楢村ほか : 透析液中の細菌に対する各種メンブレンフィルター法の測定精度の検討 緒 言 近年, 米国 ANSI/AAMI における透析液水質のガイドラインが ISO 基準案に色濃く反映され, 本邦においても細菌基準を含めた新たな管理基準の作成と臨床での細菌検査の実施が奨励されている 1,2). 一般に, 細菌検査はクリーンベンチ等を用いて慎重に作業されるが, 通常の透析施設ではそのような設備や経験はない場合が殆どである. さらに, 細菌学領域にて汎用されているメンブレンフィルター ( 以下,MF) 法 3) は, 専用のフィルターホルダーやマニホールド等を必要とし, 作業工程も増えるため簡便に実施できるとは言い難い. そのような背景から, 作業時の細菌汚染が少なく, 簡便に MF 法を施行できる製品が透析施設で汎用されている. しかし, これらの MF 法について, 透析液中に存在する細菌の測定精度を確認した報告は少ない. 今回われわれは, 各種 MF 法製品において, 透析液中に存在する細菌の測定精度について平板塗抹法との比較検討を行ったので報告する. Ⅰ. 対象および方法. 検討対象簡便な取り扱いの MF 法製品として,MF 色の異なる2 種類の 37mmクオリティモニター ( 日本ポール社製 ; 孔径 0.45 mm, 以下,QM) およびマイクロファンネル ( 日本ポール社製 ; 孔径 0.2 mm)( 図 1) を検討対象とした.QM は, 直径 37mmのMFが組み込まれたカートリッジ型の MF 法製品で, シリンジのみで検体処理が可能であり, 液体培地を用いて細菌を培養する. マイクロファンネルは, 直径 47mmのMFがファンネルに組み込まれた MF 法製品で, 液体培地ま たは寒天培地のどちらを用いても細菌培養が可能であり, 検体濾過には吸引装置が必要なものの, 一度に多量の検体を短時間で処理できる製品である. 比較対象は, 本邦で広く普及し ISO 基準案 2) にも収載されている R2A 平板塗抹法とした. また, 同じく ISO 基準案に収載されている TGEA 平板塗抹法も同時に実施した. 表 1 に比較試験に用いた細菌検出法および製品と培養方法の組み合わせを示す.. 試験菌液の調製標準菌株は, 第 15 改正日本薬局方の R2A 寒天培地の性能試験に用いられる Pseudomonas fluorescence (NBRC 15842)( 以下,P. fluorescence),methylobacterium extorquens(nbrc 15911)( 以下,M. extorquens) を使用した 4). また, 供給装置内の透析液を R2A 寒天培地で培養した後, 培地上に形成されたいくつかのコロニーから細菌を単離して試験に供した. 透析液より単離した菌株 ( 以下,Wild type) はその特徴を把握するため,( 財 ) 日本食品分析センターにおいて, 性状を調べることに加え,16S rrna の塩基配列を解析し, 近縁種を確認した 5 7). その結果,Wild type は Brevundimonas aurantiaca に最も近縁であることを確認した. 性状と MicroSeq Analysis Software を用いて近隣結合法により作成した近縁種との系統樹を図 2 に示す. これら 3 種の菌株を復元培地および適当な培地で培養し, 培養した細菌を注射用水に懸濁して cfu/ml に調製した後, インキュベーターにて 25± 2,3 日間飢餓状態で保存した 4). 保存した細菌は, 0.2 mm フィルター ( 日本ポール社製アクロバック滅菌吸引フィルターユニット ) で滅菌濾過した透析液中 に約 cfu/0.1 または 0.2 ml(1 回に注入した菌液量 ) となるように希釈調製し, 試験に供した. 図 1 日本ポール社製 37 mm クオリティモニター ( 左 ) とマイクロファンネル ( 右 )

4 88 楢村ほか : 透析液中の細菌に対する各種メンブレンフィルター法の測定精度の検討 し,MF 上に残液がなくなるまで出口側からシリンジで吸引濾過した. 続いて,2mLのTGE 液体培地を入口側から注入し, 出口側からシリンジで吸引濾過した. 出入り口の両方に蓋をした後, 培養し MF 上に形成したコロニー数をカウントした. ) マイクロファンネル ( 液体培地使用 ) マイクロファンネルの蓋を開けて 10 mlの透析液を注入し, 続いて希釈調製した菌液を 0.1 または 0.2 ml 注入した. 吸引ポンプにて吸引濾過後, 蓋を開けて2mLのR2A もしくは TGE 液体培地を注入し, 吸引残圧にて液体培地をメンブレンパットに染み込ませた. その後, シリンダー部分を外して, ベース部を蓋とキャップで密閉した後, 培養し MF 上に形成したコロニー数をカウントした. ) マイクロファンネル ( 寒天培地使用 ) マイクロファンネルの蓋を開けて 10 mlの透析液を注入し, 続いて希釈調製した菌液を 0.1 または 0.2 ml 注入した. 吸引ポンプにて吸引濾過後, ベース部からピンセットで MFを取り出し,R2A もしくは TGE 寒天培地上に貼付して培養し, 培地上に形成したコロニー数をカウントした. 培養条件は, すべて 25±2 で7 日間倒置培養とした. 各試験は, それぞれ10 回行い, 結果は平均回収率 ± SD にて表示した. Ⅱ. 結果平板塗抹法にて R2A 寒天培地上に形成したコロニー数を 100% としたときの, それぞれの手法の回収率を図 3 に示す. P. fluorescence の場合, すべての組み合わせの平均回収率で 80% 以上を示した. マイクロファンネルと QM に対して R2A 培地 ( 液体または寒天 ) を使用した場合の回収率は, マイクロファンネル (R2A 液体 ) 81.4%±14.9%, マイクロファンネル (R2A 寒天 ) 91.7%±11.9%,QM 白 (R2A 液体 )80.7%±10.4%, QM 黒 (R2A 液体 )114.4%±25.8% であった. 別の種類の寒天培地に塗布した場合の回収率は,TGE 寒天培地 119.1%±19.2% であった. また, マイクロファンネルと QM に対して TGE 培地 ( 液体または寒天 ) を使用した場合の回収率は, マイクロファンネル (TGE 液体 )85.4%±9.1%, マイクロファンネル (TGE 寒天 )81.2%±9.5%,QM 白 (TGE 液体 )88.5% ±11.8%,QM 黒 (TGE 液体 )80.9%±18.4% であっ た. M. extorquens の場合, すべての組み合わせの平均回収率で 75% 以上を示した. マイクロファンネルと QM に対して R2A 培地 ( 液体または寒天 ) を使用した場合の回収率は, マイクロファンネル (R2A 液体 ) 108.2%±14.5%, マイクロファンネル (R2A 寒天 ) 91.8%±10.2%,QM 白 (R2A 液体 )80.8%±12.8%, QM 黒 (R2A 液体 )97.8%±20.8% であった. 別の種類の寒天培地に塗布した場合の回収率は,TGE 寒天培地 92.8%±21.4% であった. また, マイクロファンネルと QM に対して TGE 培地 ( 液体または寒天 ) を使用した場合の回収率は, マイクロファンネル (TGE 液体 )75.7%±11.7%, マイクロファンネル (TGE 寒天 )81.1%±11.5%,QM 白 (TGE 液体 )75.8%± 10.2%,QM 黒 (TGE 液体 )79.7%±12.9% であった. Wild type の場合, すべての組み合わせの平均回収率で 80% 以上を示した. マイクロファンネルと QM に対して R2A 培地 ( 液体または寒天 ) を使用した場合の回収率は, マイクロファンネル (R2A 液体 )108.8% ±18.5%, マイクロファンネル (R2A 寒天 )98.7%± 14.4%,QM 白 (R2A 液体 )107.7%±17.6%,QM 黒 (R2A 液体 )80.6%±12.7% であった. 別の種類の寒天培地に塗布した場合の回収率は,TGE 寒天培地 93.0%±8.8% であった. また, マイクロファンネルと QM に対して TGE 培地 ( 液体または寒天 ) を使用した場合の回収率は, マイクロファンネル (TGE 液体 ) 81.9%±11.9%, マイクロファンネル (TGE 寒天 ) 108.7%±44.9%,QM 白 (TGE 液体 )88.9%±13.9%, QM 黒 (TGE 液体 )81.2%±10.4% であった. Ⅲ. 考察エンドトキシンを代表とする細菌由来の生理活性物質は, 生体に炎症反応をひきおこすことから, さまざまな合併症の発症に深く関与している 8). 各種洗浄消毒剤や熱水処理によるシステムの清浄化が期待通りに行われているかどうかを確認するためにも, 透析液の製造工程管理において, 細菌の汚染状態を把握し対策を講ずることが, 透析患者への悪影響を防ぐために重要である. 平板塗抹法は比較的簡便に培養細菌数を知ることが可能であるが, 検出に使用する試料が微量であるため, ある程度清浄化された透析液中の微量な細菌を検出する際には, 真の細菌現存量との間の乖離が問題となる. 平板塗抹法で細菌が検出されない場合, より高感度に細菌の汚染状態を把握するために MF 法を用いて検査することが必要である 3). しかし, 従
5 楢村ほか : 透析液中の細菌に対する各種メンブレンフィルター法の測定精度の検討 89 図 3 平板塗抹法にて R2A 寒天培地上に形成したコロニー数を 100% としたときの回収率 来の MF 法を透析施設で実施する場合, 専用の装置 設備を持っていない, 細菌検査の経験がないなど, 普及への障害が多く存在する. これらの問題を解決した簡便な MF 法は,MF とファンネルが一体化した構造であり, 特別な装置が不要であるなど, 細菌検査の経験のない者でも容易に取り扱うことが可能である. 今回われわれは, これらの MF 法と平板塗抹法との比較検討を行った. 細菌検査の測定精度は, 第 15 改正日本薬局方第一追補において, 接種菌の出現集落数は標準化された菌 液の計測値の 1/2 2 倍以内 ( 回収率 %) でなければならないと明記されており 9), 米国薬局方では少なくとも 70% 以上であることが要件とされている 10). 評価の結果, 今回用いた MF 法製品と培養方法の組み合わせは,R2A 寒天培地を用いた平板塗抹法の 細菌回収率を 100% とした場合, すべて上記の条件を満たしていた. また, マイクロファンネルにおいて, 液体培地と寒天培地の培養結果の差は殆どみられなかった. さらに QM の白色 MFと黒色 MFの間にも差はみられなかった. 細菌を飢餓状態にすると細菌の矮小化がおこることが報告されていることから 11), 孔径 0.2 mm のマイクロファンネルで細菌の回収率が良好になる可能性も考えられたが, 今回の結果からは, 孔径の違いによる細菌回収率の差はみられなかった. その理由としては以下が考えられる.MF は同じ大きさの孔が規則正しく無数に開いているスクリーン構造ではなく, 迷路のような複雑な構造によって異物を除去するデプス構造になっており, その孔径は指標となる細菌 ( 例えば,0.2 mm の場合 Brevundimonas diminuta,0.45 mm の場合
6 90 楢村ほか : 透析液中の細菌に対する各種メンブレンフィルター法の測定精度の検討 Serratia marcescens) のチャレンジ試験によって規定されている. 細菌は,MF の表面でも捕捉されるが, 複雑な厚みを持ったデプス構造の内部でも捕捉される 12). そのため, 実際には規定孔径以下の大きさの細菌でも捕捉する能力を有しており, その回収率には影響しなかった可能性が考えられる.MF の材質 メーカーの違いで細菌の発育に大きな差がみられることも知られており 13), 同じ材質でもその開孔率により発育の程度がかわることから,MF を選定する際にはデータを十分に取り, 期待する性能が得られることを確認した上で使用することが重要である. また, 操作条件による値のばらつきにも注意する必要があり,QM に関しては高加圧 高流量で検体を注入した場合でも, 細菌回収率に全く影響を与えないことを確認している. 今回用いた各種簡易的 MF 法製品は, いずれの手法においても前述の回収率要件を満たしており, 同等に用いることが可能と考えられた. 一方で,RO 水中や透析液中には, 生きているが培養困難な細菌 (VBNC: Viable But Non-Culturable bacteria) が多く存在することが知られており 14 17), 一種類の培地ですべての細菌を培養し検出することは困難である. よって, 透析液の品質を保証するには, 単に任意の測定法を用いて評価するのみでは不十分であり, 透析液製造工程における細菌の挙動を把握 予測することが重要となる. すなわち, おこりうる細菌汚染の危害をあらかじめ予測 分析し対策を講じることから始め, 各工程で細菌を定期的にモニタリングし, 結果として最終透析液が基準をクリアできるよう管理を行う必要がある. 各工程および最終透析液の細菌管理を行う際に, 今回評価した各種簡易的 MF 法は有効なツールとして十分に活用できるものと考えられた. 結語今回評価した各種簡易的 MF 法は,R2A および TGEA 平板塗抹法と同等の測定精度を有することが示され, 透析液製造工程および最終透析液の細菌管理の有効なツールとして十分に活用できることが確認された. 文献 1) 川西秀樹 : 透析液清浄化基準 (1) 国内の細菌の動向. 臨牀透析 23: ,2007 2) International Organization of Standardization.
