[Subjects] The Subjects were 18 healthy Japanese women. [Method] Subjects performed SS and DS of their triceps surae muscles. We measured ROM of ankle dorsiflexion and muscle strength of ankle plantar flexion of subjects before stretching, just after stretching, and 5 minutes and 10 minutes later. [Results] ROM of subjects who did SS increased just after stretching and this was maintained for 10 minutes. On the other hand, ROM of DS increased gradually and reached that of SS. Subjects who performed DS showed significantly greater ankle plantar flexion strength than subjects who did SS from just after the stretching to 10 minutes later. [Conclusion] These findings indicate that DS 10 minutes before performing some sporting activities in which peak muscle strength is necessary would enhance performance. 2 786 理学療法科学第 25 巻 5 号 I. はじめに スポーツ活動の前には外傷予防や競技能力向上のため関節可動域の増大を目的としてストレッチングが行われる 一般にスポーツの現場では, 筋を持続的に伸張する 静的ストレッチング (static stretching;ss) を行うことが多いが,SS 後には筋力低下が起こることが指摘されており 1,, 十分な競技能力を発揮できない可能性がある それに対して, 伸張したい筋の拮抗筋を随意的に収縮し, 相反性抑制による筋の弛緩を引き出す 3) 動的ストレッチング(dynamic stretching;ds) は筋力の低下が少ないという報告があり 4,5), より大きな筋力発揮を必要とするスポーツ活動前の実施に適していると考えられる しかし, 静的および動的ストレッチングを行い, 関節可動域と筋力という点について両者を比較した先行研究では, ストレッチング直後のみでの比較を行っているもの 1,2,4,5) が多く, ストレッチング終了後に時間の経過で比較した研究はみあたらない このことから,DS の利点や導入方法についてはこれまで十分な見解は得られていないと考える 本研究では, スポーツ外傷の多い下腿三頭筋に対し SS およびDS を実施し, その前後の足関節背屈可動域と足関節底屈筋力の経時的変化を比較することで, それぞれのストレッチングの特徴を明確にし, よりスポーツ活動前に適したストレッチング方法を明らかにすることを目的とした II. 対象と方法 1. 対象対象は, 本研究の趣旨に賛同の得られた成人女性 18 名の右下肢とした 対象はいずれも下肢に整形外科疾患がない者とした 年齢 ( 平均 ±SD) は 20.3 ± 1.5 歳, 身長は159.2 ±5.1 cm, 体重は 53.2 ±4.4 kg であった なお, 本研究は広島大学大学院保健学研究科心身機能生活制御科学講座倫理委員会の承認を得て行った ( 承認番号 0848) 2. 方法対象者全員に,2 種類のストレッチング (SS,DS) とストレッチング非実施 (non stretching;ns) の3 つの条件を課し, ストレッチングの直前, 直後,5 分後,10 分後に足関節背屈可動域および足関節底屈筋力を測定した 3 つの測定条件は対象毎にランダムに行い, それぞれの測定は, 他のストレッチングの影響を排除するた 図 1 自作のストレッチングボードめ, 先行研究 6) を参考に7 日間以上あけて行った 足関節背屈可動域の測定には, 巻き上げねじによって傾斜角度の調節可能な自作のストレッチングボードを使用した ( 図 スポーツ活動は立位で行うことが多く, 立位での可動域がより重要であると考え, 立位で可動域測定を行っている先行研究 6) を参考に本装置を作製した なお, 足関節背屈に伴う踵部への圧迫による疼痛を考慮し, 足部後方に2 cm 幅のクッションを設置した 対象にはストレッチングボード上で膝伸展位, 荷重が両下肢均等となるような立位をとらせた 踵部がストレッチングボードから離れるか, 股関節または膝関節が屈曲するか, あるいは足関節周辺に疼痛を生じる直前の角度を側方よりデジタルカメラで撮影した 床面とストレッチングボードに反射マーカーを貼付し, それぞれのなす角を足関節背屈角度とし, 画像解析ソフトScion Image(Scion 社 ) により足関節背屈角度を算出した 足関節底屈筋力の測定肢位は背中を壁につけた長座位とした 足関節底背屈 0º にて, 中足骨遠位部にベニヤ板, ハンドヘルドダイナモメータ (μtas F1,ANIMA 社 ) を装着し, 骨盤帯と足部をベルトで固定し, 最大努力で 2 秒間下腿三頭筋の等尺性収縮を行わせた ( 図 2 回の練習後,3 回測定を行い, 最大値を採用した SS は, ストレッチングボードに両下肢で乗り, 静的ストレッチングによって腓腹筋が有意に伸張されるという報告をもとに2 分間静止立位をとらせた 7) このときの傾斜角度は, ストレッチング前に測定した足関節最大背屈角度とした DS は, 長座位にて実施した ベッドの足に固定されたチューブ (Thera-Band ブラック, Hygenic 社 ) を中足骨背側部にかけ, 足関節最大底屈位から最大背屈位までの前脛骨筋による自動運動を,1 Hz のリズムで20 回繰り返させた 立位では膝関節を伸
3 静的および動的ストレッチング後に生じる足関節可動域と筋力の経時的変化 787 III. 結 果 図 2 足関節底屈筋力測定方法展位で下腿三頭筋を伸張し, 角度測定を行っているので,DS も長座位とした 測定値は平均 ± 標準偏差で示した 統計処理については, エクセルアドインソフト Statcel2( オーエムエス出版 ) を用いた 足関節背屈可動域と足関節底屈筋力の経時的変化については, ストレッチング直前の値からの差 ( 直後,5 分後,10 分後 ) と3 条件 (NS,SS,DS) を 2 要因とした反復測定二元配置分散分析を用いて検討し, 有意な効果が得られた場合には,Fisher の PLSD 法による多重比較を行った いずれも危険率 5% 未満を有意とした また, 可動域および筋力の測定結果の再現性は級内相関係数 (ICC) を用いて検討した 測定結果の再現性については可動域, 筋力ともにICC 0.93 という高い再現性が示された 表 1 に足関節背屈可動域の推移とストレッチング直前との差を示した ストレッチング直前では,NS,SS, DS の可動域に有意な差は認めなかったが,SS および DS 後はどちらも直後から10 分後にかけて背屈可動域が有意に増大した (p<0.05) ストレッチング直前の角度からの変化量で比較すると, ストレッチング直後は, DS の1.8 ±1.3º に対し,SS で3.5 ±3.3º とSS が約 1.7º 大きい値を示した (p<0.05) しかし,SS はその後の背屈可動域に変化がなかったのに対し,DS では 10 分後まで徐々に増大し,10 分後にはSS が3.8 ± 3.6º,DS が3.4 ± 1.5º の背屈可動域増大となり, 両者における有意差はみられなくなった 表 1 に足関節底屈筋力の推移とストレッチング直前との差を示した ストレッチング直前では,NSが586.4N と SS(666.7 N),DS(643.1 N) より小さいが, 筋力に有意な差は認めなかった (p>0.05) ストレッチング直前との差をみると,SS では直後に 22.0 ±61.3 N,10 分後に 30.4 ± 87.1 N と低下した 一方,DS では 20.5 ± 66.7 N,10 分後に19.3 ±56.9 N と増加したが,NS を含む全てでストレッチング直前から10 分後の全てにおいて有意な差は認められなかった しかし,SS とDS を比較すると, ストレッチング直後から10 分後にかけて,SS はDS よりも有意に低い値を示した (p<0.05) 表 1 足関節背屈可動域および底屈筋力の推移 (( ) 内は直前からの変化量を示す ) 直前直後 5 分後 10 分後 足関節背屈可動域 NS 27.8 ± ± ± ± 5.1 (º) ( 0.5 ± 2. 4 788 理学療法科学第 25 巻 5 号 IV. 考察本研究ではストレッチングなし (NS), 静的ストレッチング (SS) および動的ストレッチング (DS) の直前からストレッチング終了 10 分後までの足関節背屈可動域と底屈筋力を測定した SS とDS という異なる様式のストレッチングで, 同等とみなされるストレッチングを実施することは困難であったが, 先行研究や実際のスポーツ現場の様子を参考に, ストレッチングを行う時間や運動量など関節可動域の拡大には十分であると思われる条件で実施した 足関節背屈可動域が増大しなかったNS に対し,SS およびDSではストレッチング直前と比較してストレッチング終了直後から終了 10 分後まで可動域の増大を認めた さらに,SS とDS を比較すると可動域の推移に差を認めた ストレッチング終了直後,SS はDS に対してより大きな可動域の増大を認めた この結果について, それぞれのストレッチング方法の特徴から考察する SSは, 立位で筋に他動的かつ持続的な伸張を加えた これに対しDS は, 座位で筋に自動的かつ中等度の速度でのリズミカルな伸張を繰り返し加えた 本研究と同様の方法でSS を行い, 筋および腱に対する効果を超音波画像で分析している先行研究によると 7),SS では腓腹筋の腱よりも筋の方がより伸張されることが明らかになっている 一方 DS では下腿三頭筋に対する伸張は SS よりも小さく, 腱の伸張性や隣接組織に対する筋腱の滑走性が向上するとされている 8,9) 競技中の外傷予防のためには, 腱の伸張性や隣接組織の滑走性も必要であるが, 筋の伸張性を十分に高め, 可動域を増大させるという点においてDS 直後は不十分な状態なのではないかと考える しかし, 本研究によってDS ではストレッチング直後から10 分後にかけて可動域が増大することが明らかになった 筋の収縮および弛緩を繰り返すと, 筋のポンプ作用や交感神経の働きによって筋の血液循環が高まる 血流量が増加して筋温が上昇することによって, 筋硬度が低下し伸張性が向上する 10,1 DS 終了後に可動域がDS の特徴である自動運動を繰り返すことが, 筋を直接伸張するのみでなく血液循環を促進し筋の伸張性を向上させるのであれば,DS の利点であると思われる DS 終了直後には不十分である背屈可動域の増大効果が, ストレッチング終了 10 分後でSS と同程度まで向上するという本研究の結果により,DS を行う際には競技開始の10 分前に行うことで, 競技遂行により適した状態となる可能性が示された もちろんSS でもスト レッチング終了後に筋の血液循環が高まる可能性が考えられるが, このような視点から今後の調査を試みる意義はあると考える 足関節底屈筋力は, ストレッチング終了直後から10 分後まで SS に対して DS が有意に高い値を示した SS 1, 直後に筋力発揮が低下するという結果は先行研究通りであり, その理由として Ib 抑制の影響が示されている 下腿三頭筋が持続的に伸張されたことにより同筋の腱紡錘が興奮し, 筋収縮が抑制される 10) また, 本研究ではSS 後の筋力低下がストレッチング終了 10 分後でも持続するということが明らかになった このような結果となった要因として, 筋の長さと張力の関係が考えられる 筋は静止長を超えて伸張されると, 一旦張力が低下する 1 SS はDS と比較して筋伸張の要素が大きいため, 下腿三頭筋の一時的な筋出力低下が生じやすかったと推察される 一方,DS では筋力が増加傾向を示した この理由として, 筋温の上昇の可能性を考える 13) 筋温が上昇すると, 筋の粘性の低下により収縮が滑らかになることや, 神経刺激の伝達速度が上昇することなどがいわれている 14) 前項の筋血流量の増加の可能性と同様に SS でも筋温の上昇が起こるか, 調査する必要があると考える 上述した結果より, 下腿三頭筋に対するDS はスポーツ活動の10 分前に行うことで筋力発揮の低下を起こすことなく, かつSS と同程度の関節可動域の増大が得られることが示された DS が直接競技能力向上につながるとはいいきれないが, 特に高い筋力が必要とされる競技の前に DS を行うことで SS 後よりも高い筋力を発揮し, 競技能力が向上する可能性が考えられる 本研究では, 下腿三頭筋に対し2つの異なるストレッチングを実施し, その後の可動域と筋力についての比較を経時的に行うことで, 今まで明らかになっていないSS とDS 後における差を発見するに至った しかし, 本研究では今回明らかとなった SS と DS の差が生じた理由に関しては明確にはなっておらず, この理由の解明には筋血流量や筋温といったより詳細な情報が必要となるため, 今後の課題としていきたい また, 本研究では 10 分後以降の可動域や筋力は測定していない DSはストレッチング10 分後まで可動域が増大し続けており, さらに増大する可能性も考えられる より効果的に DS を取り入れる方法を知るためには,DS 後に可動域が最大になるまでに要する時間や,SS 後に生じる一時的な筋力低下が消失する時間を明らかにするためにさらなる研究を進める必要がある さらに, 今回の実験ではそれぞれのストレッチングが可動域や筋力発
