1 日外科系連会誌 39(6): ,2014 症例報告札幌東徳洲会病院乳腺外科 1) 2), 同病理診断科 1) 2) 南盛一長嶋和郎内容要旨症例は84 歳男性. 約 2 年前より左乳房腫瘤を自覚していたが放置. 吐血のため当院に救急搬送され, 上部消化管出血の精査目的に上部消化管内視鏡検査を行った. 胃噴門部に約 5cm 大のBorrmann2 型の腫瘍を認め, 生検にてtub2の結果であり胃癌の診断となった. また, 入院後の胸部 CTで左乳房に 3.5cm 大の腫瘤を認め, 乳房超音波検査で低エコー腫瘤として描出され乳癌を否定できなかったため針生検を施行した. 病理学的に浸潤性乳管癌と診断され, 各々の転移ではなく左乳癌, 胃癌の同時性重複癌と判断した. 手術は乳癌, 胃癌の順に逐次行い, 術後補助療法は, 乳癌に対してはタモキシフェン内服によるホルモン療法を行っているが, 認知症のため胃癌に対する補助化学療法は行わず経過観察中である. 男性乳癌は全乳癌の約 1% とされ比較的稀で, 同時性に胃癌と重複した報告例は本症例が8 例目と稀であり, 文献的考察を加えて報告する. 索引用語 : 男性乳癌, 胃癌, 同時性はじめに男性乳癌はこれまで全乳癌症例の約 1% とされてきた比較的稀な疾患である. 今回われわれは男性乳癌と胃癌との同時性重複癌の稀な1 例を経験したので文献的考察を加えて報告する. 症例症例 :84 歳, 男性. 主訴 : 吐血. 家族歴 : 特記事項なし. 既往歴 : 脳梗塞, 認知症. 現病歴 : 約 2 年前より左乳房腫瘤を自覚していたが放置していた. 平成 25 年 10 月, 自宅で吐血し, 当院に救急車で搬送され入院となった. 入院時現症 : 腹部は平坦, 軟で腫瘤を触知せず. 受付 :2014 年 4 月 4 日, 採用 :2014 年 6 月 25 日連絡先南盛一北海道札幌市東区北 33 条東札幌東徳洲会病院乳腺外科左乳房 E 領域に cmの弾性硬の腫瘤を触知した. リンパ節は触知しなかった. 入院時血液検査 : 末血ではHb12.4g/dlと軽度の貧血を認めた. 生化学検査, 腫瘍マーカーは正常範囲内であった. 上部消化管内視鏡検査 : 胃噴門部直下から胃体中部前壁に径約 5cmのBorrmann2 型の腫瘍を認め, 周堤には露出血管を伴っていた (Fig. 1 ). 腫瘍は易出血性であったが, 凝固法により止血された. 上部消化管造影検査 : 胃噴門から胃体上部前壁に5cmにわたる不整な隆起性病変を認めた (Fig. 2 ). 乳房超音波検査 : 左乳房 E 領域に mm の境界明瞭で内部は比較的均一な低エコー腫瘤を認めた (Fig. 3 ). 胸腹部 CT 検査 : 左乳輪直下に28 29mm 大で内部が均一なsoft tissue densityの腫瘤を認めた. 肝転移, 肺転移, リンパ節腫大は認めなかった (Fig. 4 ). 乳腺腫瘍針生検 : 一部に腺管形成とcomedo 1076

胃腫瘍生検 : 潰瘍に接して異型腺管を形成する tubular adenocarcinoma(tub2) の所見を認めた. 経過 : 以上より, 左乳癌 Stage ⅡA, 胃癌 Stage Ⅲbの診断となったが, 胃癌からの出血は止まっていたため緊急性はないと判断し, 侵襲の比較的小さな乳癌の手術を先行した.

Fig. 3 Breast ultrasound: A relatively homogenous hypoechoic mass with a well-defined margin was detected in the E area of the left breast. typeの壊死がみられる浸潤性乳管癌の所見であった. 胃腫瘍生検 : 潰瘍に接して異型腺管を形成する tubular adenocarcinoma(tub2) の所見を認めた. 経過 : 以上より, 左乳癌 Stage ⅡA, 胃癌 Stage Ⅲbの診断となったが, 胃癌からの出血は止まっていたため緊急性はないと判断し, 侵襲の比較的小さな乳癌の手術を先行した. 平成 25 年 10 月, 左乳房切除および腋窩リンパ節郭清を行い, 乳癌の手術から30 日後に胃全摘術 D1 郭清,Roux-en-Y 再建を施行した. 乳癌の術後病理組織所見は, cmの充実腺管癌の所見であった (Fig. 5a ). リンパ節転移, 脈管侵襲はなく, 組織学的 Grade 2, ERおよびPgR 陽性,HER2 陰性,CK7 陽性,CK20 陰性であった. 胃癌については, cmのtub2>por2 (Fig. 5b ) で深達度 SE, リンパ節に転移を認めた (U,less,Type2,45 45mm,tub2>por2,int, INFβ,ly1,v1,T3,N3a(9/13),cM0,pStage Ⅲc,pPM(-),pDM(-)).ERおよびPgR 陰 1077

Fig. 5 a: (Left) Histopathological findings of breast cancer: There were findings of invasive ductal breast carcinoma, especially of solid tubular carcinoma (HE 100). b: (Right) Histopathological findings of gastric cancer: Adenocarcinoma cells (tub2>por2) were noted in the region from the mucosal surface, extending to all layers (HE 100). 性,HER2 陽性,CK7 陰性,CK20 陰性であった. 免疫組織学的に乳癌と胃癌では発現が異なっておりどちらかの転移性の癌ではなく重複癌と診断された. 術後の補助療法は, 乳癌についてはタモキシフェン内服によるホルモン療法を開始した. 胃癌については年齢,ADLを考慮して補助療法は行わず経過観察のみとしたが現在まで再発は認めていない. 考察本邦における男性乳癌の頻度は全乳癌の約 1% とされてきた 1). しかし, 日本乳癌学会乳癌登録集計によると,2004 年から2011 年を通じて0.4~0.5% と低率であり, 現在の男性乳癌の全乳癌に対する頻度はこれまで言われてきた1% より低い可能性 1078

4 Table 1 Case reports of synchronous gastric and male breast cancers in Japan Case Author Stage Stage gastric breast Year age surgical treatment surgical treatment cancer cancer pathological findings pathological findings prognosis Yamazaki StageⅠa StageⅠ yes yes alive 21 days adenocarcinoma ductal carcinoma Shida StageⅠa StageⅢ yes yes alive 10months well diff. adenocarcinoma medullary carcinoma Nakagoshi StageⅡ StageⅡ yes yes alive 3years 1months papillary adenocarcinoma ductal carcinoma in situ Takeuchi StageⅣa StageⅠ yes yes alive 6months moderatelly diff. adenocarinoma medullary carcinoma Sato StageⅡ StageⅡ yes yes alive 9months papillary adenocarcinoma solid-tubular carcinoma Kawabuchi StageⅡ StageⅠ yes yes alive papillary adenocarcinoma papillotubular carcinoma Aoyagi StageⅢa StageⅢa yes yes alive 21 days poorly diff. adenocarcinoma invasive ductal carcinoma our case StageⅢc Stage Ⅱa yes yes alive 6months moderatelly diff. adenocarinoma solid-tubular carcinoma が高いと考えられる. 一方, 日本癌治療学会用語 ICD-11 委員会編用語集 (2013 年版 ) によると, 重複癌とは, 異なる臓器にそれぞれ原発性の癌が存在するものであり, 重複癌が発生する頻度は1~ 5% 程度との報告が多い 2). このように, 発症すること自体稀である男性乳癌症例に胃癌が同時性に重複して発症することは極めて稀であり, 本邦でも本症例が8 例目と非常に少ない (Table 1 ). 8 例の報告例に関する臨床病理学的な一定の傾向は見いだせなかった. 同時性に胃癌と重複する臓器しては大腸癌, 食道癌, 腎癌の順に多いとされ, 胃癌に重複癌が発生する頻度は2.0~5.0% と報告されている 3). 一方, 女性において重複する癌として最も多いのは乳癌である. 胃癌と乳癌が同時性重複癌か, 転移性かを判断するには画像所見や肉眼型で確定できないことが多く, 病理組織検査が必要となる 4).HE 染色,ER, PgRの他,CK7,CK20の免疫染色が診断に有用である. すなわち, 乳癌は,CK7 陽性 /CK20 陰性のパターンになることが多いのに対して胃癌では CK7/CK20の発現パターンが一定ではなく,CK7 もCK20も部分的に染色されるのが多いのが特徴である 5)6). 本症例でも,ER,PgR,CK7,CK20の結果は対照的で, 診断には免疫染色が非常に有用であった. 男性乳癌の治療に関しては, 女性の乳癌と同様に方針を決めているのが現状である. 女性乳癌におけるER 陽性率が40~60% であるのに対して男性乳癌では80% 以上と高率である 7). 一方,HER2の過剰発現は女性乳癌に比べて低いとの報告が多く 8), 本症例でもER,PgR 陽性,HER2 陰性であった. 本症例では男性にもエビデンスのあるタモキシフェン内服による内分泌療法を行うこととした 9)10). 胃癌の治療に関しては, 胃癌治療ガイドラインで推奨されているように, 手術による切除が基本だが, 分化型で2cm 以下のものに対しては内視鏡的切除が適応とされ,pStage Ⅱ,Ⅲの症例に対しては手術後の補助化学療法が考慮される. 本症例はpStage Ⅲcであったが, 年齢や痴呆などを考慮して補助化学療法は行わず手術単独の治療となった. 1079

He was transferred by ambulance to our hospital due to hematemesis and underwent upper gastrointestinal tract endoscopy for detailed examination of upper gastrointestinal hemorrhage. A Borrmann type Ⅱ tumor approximately 5cm in diameter was detected in the gastric cardia. Biopsy showed a moderately differentiated tubular adenocarcinoma (tub2). Therefore, gastric cancer was diagnosed. Chest computed tomography (CT) after admission showed a 3.5cm mass in the left breast, and breast ultrasound showed a hypoechoic mass. Needle biopsy gave a pathological diagnosis of invasive ductal breast carcinoma. The cancers were judged to not be metastases from other cancers but rather to be synchronous double cancers of the left breast and stomach. Breast cancer surgery was performed first, after that gastric cancer surgery was done, followed by hormone therapy with oral tamoxifen as postoperative adjuvant therapy. The gastric cancer has been observed without adjuvant chemotherapy because the patient has dementia. Male breast cancer is relatively rare, accounting for only 1% of all breast cancers. This is the 8 th reported case of male breast cancer developing simultaneously with gastric cancer, which is also rare. We report this case with discussion of the relevant literature.
