能である 47 d. 眼窩下神経ブロック 眼窩下神経は眼窩下孔から出るので 眼窩下孔内にブ ロック針を刺入し 薬液注入または高周波熱凝固を行う Fig. 5 に眼窩下神経ブロック時の写真を示す 適応は三叉神経第 2 枝の三叉神経痛である X 線透視は必要ない アルコールを用いた場合は穿刺部位近傍の浮腫が高頻 度で出現し 1 週間で消失する その他の合併症は眼窩 内に刺入した際の外眼筋麻痺 壊死 上顎洞穿刺がある Fig. 5 外眼筋麻痺を防ぐために 局所麻酔薬の試験投与時に複 眼窩下神経ブロック 視がないことを確認する e. 下顎神経ブロック 窩下神経となる 翼口蓋窩に穿刺針を刺入し上顎神経を ブロックする X 線透視下に行う 下顎神経は卵円孔を出て 前枝と後枝に分かれる 前 枝は咬筋 翼突筋を支配し 後枝は耳介側頭神経 下歯 眼窩下神経ブロックでは疼痛が軽減しない臼歯部や側 槽神経 舌神経に分かれる 卵円孔を出た部位に穿刺針 頭部の疼痛が適応となる しかし 他の神経ブロックよ を位置させてブロックを行う X 線透視が必要である り手技が困難で成功率が低く 合併症の頻度が高い 眼 穿刺時の疼痛は強いので 軽度の鎮静も考慮する 窩内を穿刺した際には失明 視神経障害 外眼筋麻痺が 適応は第 3 枝の三叉神経痛である アルコールを用い 生じる 近年上顎神経の破壊的ブロックは行われず よ ることもあるが 近年では高周波熱凝固が選択されるこ り安全な Gasser 神経節高周波熱凝固が選択されている とが多い Gasser 神経節内に位置させた穿刺針で電気刺激を行い Gasser 神経節内の上顎神経近傍に針先を位置させるこ 合併症は少なく 血腫 耳管穿刺 咀嚼筋麻痺などが ある とである程度上顎神経を選択的にブロックすることが可 ventral right left block needle dorsal foramen ovale Fig. 6 下顎神経ブロック 針の先端は卵円孔に近接している 標準的神経治療 三叉神経痛 121
