1 上原記念生命科学財団研究報告集, 27 (2013) 141. 子宮内膜症の病態の解明と新しい薬物療法の開発奈須家栄 Key words: 子宮内膜症, エピジェネティクス, マイクロ RNA, 細胞増殖, アポトーシス大分大学医学部産科婦人科学緒言子宮内膜症は全女性の3~10% に発生し, 慢性骨盤痛, 月経痛, 性交痛や不妊などの症状を呈するエストロゲン依存症の良性疾患である 1). そのため子宮内膜症を有する女性の quality of life は著しく低下する. 良性疾患でありながら悪性腫瘍と同様に転移や播種, 再発を起こし, 炎症や癒着, 瘢痕を形成するため, 子宮内膜症は類腫瘍性疾患, 類炎症性疾患と位置付けられている 2,3). 子宮内膜症は年々増加しているが, その病因, 病態形成のメカニズムに関しては未だ不明な点が多い 4-6). 近年, 子宮内膜症において DNA のメチル化, ヒストンのアセチル化といったエピジェネティクス異常が, その発症に関与していることが報告されている 7,8). 今回, 子宮内膜症の病因としてのエピジェネティクス機構の解明のひとつとして,microRNA (mirna) の役割について検討した 9). 方法および結果 1. 子宮内膜症細胞および正常子宮内膜細胞の分離培養 4,9) 卵巣子宮内膜症性嚢胞に対する手術時に, 患者より文書による同意を得て子宮内膜症組織を採取した 4). また対照として, 子宮筋腫に対する子宮全摘術の際に, 患者より文書による同意を得て正常子宮内膜組織を採取した. なお, 本研究は, 平成 22 年 7 月に大分大学医学部ヒトゲノム研究倫理審査委員会の審査に基づく許可を受けた. 子宮内膜症組織および正常子宮内膜組織を collagenase 処理し, 卵巣子宮内膜症間質細胞 (endometriotic cyst stromal cell: ECSC) および正常子宮内膜細胞 (normal endometrial stromal cell: NESC) を分離培養し, 以下の in vitro の実験を行った. 2. マイクロアレイによる遺伝子発現異常の網羅的解析 9) 培養子宮内膜症細胞および培養正常子宮内膜細胞から total RNA を抽出し,miRNA マイクロアレイ (G4470C, Human Rel. 12.0, Agilent Technologies) を用いて,miRNA 発現の網羅的解析を行った. その結果, 正常子宮内膜間質細胞と比較して, 子宮内膜症間質細胞では 8 つの mirna が減少し,4 つの mirna が増加していることが分かった (Table 1). 1

The EST transcript and the putative transcription start site are also shown. まず子宮内膜症間質細胞における mir-196b および HOXA10mRNA 発現を real-time PCR により確認したところ正常子宮内膜間質細胞と比較して子宮内膜症間質細胞において mir-196b の発現が増強していることが確認できた (p<0.01, Mann-Whitney U test) (Fig. 2) 同様に HOXA10 の発現も子宮内膜症間質細胞において増強していた (p<0.001, Mann-Whitney U test) (Fig. 3A) さらに mir-196b と HOXA10 mrna の発現の相関について検討したところ強い相関を認めた (Pearson's correlation coefficient : r=0.678, p<0.005) (Fig. 3B) 2

The data are presented as percentages of relative to the values of ECSCs transfected with negative control precursor mirna. *p<0.005 vs. negative controls (Student t test). ６培養子宮内膜症間質細胞の c-myc および Bcl-2 発現に対する mir-196b の作用 9) mir-196b の downstream target gene として c-myc および Bcl-2 に着目しこれらの遺伝子発現に対する mir-196b の作用について検討したその結果 mir-196b の強制発現により子宮内膜症間質細胞の c-myc および Bcl-2 発現は有意に抑制されることが分かった (p<0.05, Student t test) (Fig. 5A, B) mir-196b による子宮内膜症間質細胞の細胞増殖および apoptosis の制御は c-myc および Bcl-2 の発現調節を介していることが示唆された 4

The data are presented as ratios relative to the values of untreated controls. *p<0.05 vs. negative controls (Student t test). ７子宮内膜症間質細胞における mir-196b および HOXA10 mrna 発現に対する DNA 脱メチル化剤の作用 9) 子宮内膜症間質細胞における mir-196b および HOXA10 mrna の発現抑制機序について検討した子宮内膜症間質細胞を DNA 脱メチル化剤 5-aza-2'-deoxycytidine (5-aza-dC, Sigma-Aldrich) (10μM)で 4 日間刺激したところ mir-196b および HOXA10 mrna の発現は有意に増強することが分かった (p<0.05, Student t test) (Fig. 5C, D) ８子宮内膜症間質細胞および正常子宮内膜間質細胞における mir-196b 遺伝子の CpG island のメチル化に関する検討 9) 子宮内膜症間質細胞および正常子宮内膜間質細胞における mir-196b 遺伝子の CpG island のメチル化状態について methylation-specific PCR を用いて検討したその結果正常子宮内膜間質細胞では mir-196b 遺伝子の CpG island はメチル化されていないことが分かった (Fig. 6) 一方子宮内膜症間質細胞では mir-196b 遺伝子の CpG island はメチル化されていた (Fig. 6) 子宮内膜症間質細胞における mir-196b の発現抑制は CpG island のメチル化を介するエピジェネティクス機構によって生じていると考えられた 5

U, unmethylated alleles; M, methylated alleles. 考察本研究から,miRNA の発現異常が子宮内膜症の病態形成に関わっていること, 子宮内膜症間質細胞における mir-196b の発現は mir-196b 遺伝子のメチル化によって抑制されていること, さらに,miR-196b の発現抑制は, 増殖能の増強や apoptosis 耐性という子宮内膜症に特徴的な性質の形成に関与していることが示唆された. 今後,miRNA を含むエピジェネティクス機構についてさらに検討を重ねることにより, 子宮内膜症の病態形成のメカニズムが解明され, 新たな治療法や biomarker の開発, さらにはハイリスク群の診断と予防法の開発につながることが期待される. 共同研究者本研究の共同研究者は, 大分大学医学部産科婦人科学の阿部若菜, 川野由紀枝, 楢原久司および大分大学医学部分子病理学の中田千里, 守山正胤である. 本稿を終えるにあたり, 本研究を御支援いただきました上原記念生命科学財団に深く感謝申し上げます. 文献 1) Nasu, K., Tsuno, A., Yuge, A., Kawano, Y. & Narahara, H. : Combined oral contraceptives for the medical treatment of endometriosis-associated pain.
