1 原著日集中医誌.2010;17:33 ~38. 深在性真菌症の診断治療における β-d- グルカン値の評価 *1 *1, 2, 5 *3 藤木早紀子志馬伸朗廣瀬有里 *4 *3, 5 小阪直史藤田直久要約 : 目的深在性真菌症の診断における β-d- グルカン (BDG) の測定意義と適正な適用評価法を考察した方法単一大学病院の感染症検査室および薬剤部データベース解析により, 年に培養検査にて同定された深在性真菌症診断時の BDG 値の使用価値を評価したさらに,2005 年度の抗真菌薬の使用開始理由とコストを調査した結果 1Receiver operating characteristic curve(roc 曲線 ) 解析の結果, 真菌血症時の BDG の cutoff 値は 11 pg ml 1 であった 2 感度 76.1%, 特異度 78.7%, 陰性予測率 99.7% であった 3 抗真菌薬使用開始理由で,BDG を含む血清診断は 24.4% を占めた抗真菌薬使用患者のうち監視培養なし, もしくは監視培養指数 0 の症例を合わせると 81.8% を占めた 4 抗真菌薬コストは, 広域スペクトラム抗菌薬コストに匹敵した結論 BDG は陰性予測率が高いため, 除外診断検査として有用であるしかし, 過剰診断治療と支出増につながる危険性があり, 患者背景や監視培養指数を加味した適応評価が必要である Key words: 1deep-seated mycoses, 2β-D-glucan, 3antifungal agent, 4colonization index 序論 ICU 入室患者は, 広域スペクトラム抗菌薬の頻用, 悪性腫瘍に対する化学療法の強化, 移植医療の普及, 中心静脈カテーテルの使用, 血液浄化療法の施行など, 真菌感染症の危険因子を複数有しているこのため ICUにおいて真菌感染症は, 発生頻度が高く不良予後に関連する重要な合併症である 1) 真菌感染症の診断のゴールドスタンダードは真菌培養であるが, 真菌培養には時間を要すること, 深在性真菌症では検体採取が必ずしも容易でないこと, 重症例ではその見逃しと初期治療の失敗が不良予後に繋がることなどの問題がある 2) この問題を解決する一つの補助手段として, 我が国ではβ-D- グルカン測定に代表される血清診断が用いられてきた 3),4) しかし,β-D- グルカン測定のこの論文は今号のハイライトで取り上げています吉田耕一郎, 二木芳人. 深在性真菌症診断とβ-D- グルカン値測定. 日集中医誌 2010; 17: 1-3. 有用性については偽陽性など長く議論の対象となっており, その診断意義には依然として検討の余地がある 2),3),5) 9) 今回我々は, 当院感染症検査室および薬剤部におけるデータベースを活用し,β-D- グルカン値と真菌培養の結果を解析することで, 深在性真菌症診断におけるβ-D- グルカン測定の有用性を評価すると共に, 抗真菌薬の使用状況分析から, 治療開始におけるβ-D- グルカン測定の適正使用を評価した対象方法 1) 真菌感染症の発症状況 1998 年 1 月から2005 年 12 月までの8 年間に当院で行われたすべての培養検査のデータベースを用い, 深在性真菌症の発症状況を調査した無菌検体 ( 血液, 髄液, 胸水, 腹水など ) から真菌が分離同定された症例と, 下気道検体からAspergillus speciesが分離同定された症例を深在性真菌症と評価し, その症例数, 菌種別割合を調べた特に, 血液からの真菌分離症例を *1 京都府立医科大学大学院医学研究科麻酔科学, *2 同集中治療部, 受付日 2008 年 8 月 7 日 *3 同臨床検査部, *4 同薬剤部, *5 同感染対策部採択日 2009 年 6 月 24 日 ( 京都府京都市上京区梶井町 465) -33-

2 日集中医誌 J Jpn Soc Intensive Care Med Vol. 17 No. 1 真菌血症として別に評価した 2) 真菌感染症とβ-D-グルカン値同時期に当院で検査された血液培養を含むすべての無菌検体と下気道検体の真菌培養検査の中で,β-D- グルカンが同日に検出された検体のみを対象とし, 前述の基準にて深在性真菌症あるいは真菌血症にあてはまる検体とそうでない検体について,β-D- グルカン値を比較した真菌血症診断時におけるβ-D- グルカンのcutoff 値をreceiver operating characteristic curve(roc 曲線 ) 解析により求めた次にこのcutoff 値を用いて, 深在性真菌症および真菌血症におけるβ-D- グルカンの感度, 特異度, 陽性予測率, 陰性予測率を算出した 3) 抗真菌薬の使用状況と診断法当院では2005 年 8 月より, 抗真菌薬使用開始届出制度が導入されたそこで届出情報に基づき,2005 年 8 月から2006 年 7 月までの1 年間に抗真菌薬が使用された症例の抗真菌薬使用開始理由を調査したなお当院の届出制度は, 処方医が抗真菌薬の使用開始時に電子カルテ上でその開始理由を選択肢 ( 臨床診断, 血清診断, 細菌学的診断, 画像診断, その他 ) から選ぶシステムをとっており, 抗真菌薬使用開始理由はその選択肢を集計し解析した次に, これらの症例について2005 年 8 月から2006 年 7 月までの培養検査のデータベースより, 個々の患者での監視培養指数 (colonization index) を調べ, 抗真菌薬治療開始時の診断方法について考察した監視培養指数は Pittet らの報告に基づき, 身体の各部位からの監視培養サンプル数を分母にし, 真菌が同定されたサンプル数を分子として算出した 10) また, 当院薬剤部で年度毎にまとめられるデータを基に,2005 年 4 月から2006 年 3 月の1 年間に当院において使用された抗真菌薬の種別割合およびコストを算出した比較のために, 同時期において当院で処方された広域スペクトラム抗菌薬のコストを算出した 4)β-D-グルカンの測定当院におけるβ-D- グルカンの測定は,β グルカンテストワコー ( 和光純薬工業, 大阪 ) による比濁法にて行った本製品の cutoff 値は 11 pg ml 1 である 11) 5) 説明と同意本研究は院内の検査データベースを利用した後方視的検討であり, 患者への直接的な介入や研究による治療上の影響は全くないことから, 患者への説明と同意は省略した Table 1 Causative pathogens of deep-seated mycoses Species of fungi n (%) Candida albicans 90 (44.8) Candida tropicalis 23 (11.4) Candida parapsilosis 20 (10.0) Candida glabrata 9 ( 4.5) Candida guilliermondii 8 ( 4.0) Candida famata 3 ( 1.5) Candida krusei 2 ( 1.0) Candida species 8 ( 4.0) Aspergillus species 11 ( 5.5) Trichosporon species 13 ( 6.5) Torulopsis glabrata 8 ( 4.0) Others 6 ( 3.0) Total 201 (100) The most frequent causative pathogen was Candida albicans. The percentages of fluconazole resistant pathogens including Candida glabrata and Candida krusei was relatively low. 結 1) 真菌感染症の発症状況前述の8 年間に当院で行われた無菌検体の培養検査は全部で35,709 検体であった前述の診断基準により深在性真菌症と診断された症例は201 症例で, このうち真菌血症は116 症例 (57.7%) 存在した深在性真菌症の起炎菌種別割合をTable 1に示した Candida albicansが全体の44.8% と最も多く,Candida tropicalisが11.4%,candida parapsilosisが10.0% と続いたフルコナゾール耐性菌であるCandida glabrata は 4.5%,Candida krusei は 1.0% であった 2) 真菌血症とβ-D-グルカン値 1998 年から2005 年までの8 年間に提出された全血液培養提出検体 (22,347 検体 ) のうち, 培養陽性は 2,693 検体 (12.1%) であり, そのうち真菌血症の割合は171 検体 ( 培養陽性例の6.3%) であった血液培養とβ-D- グルカン測定が同時に行われていたのは8,017 検体であり, そのうち71 検体で真菌培養が陽性 ( 真菌血症 ) であったこのデータを用いβ-D- グルカンのcutoff 値を変化させて感度, 特異度を求め, ROC 曲線を描いた Cutoff 値は, 検査キットの陽性基準である11 pg ml 1 の他に,25,50,100,1,000 pg ml 1 を用いた (Fig. 1) ROC 曲線上に引かれた対角線との交点は,cutoff 値を11 pg ml 1 とした場合に最も近く, 検査キットの推奨 cutoff 値が11 pg ml 1 であることの妥当性が確認された果 -34-

Serodiagnosis results were chosen for the reason of initiating therapy more frequently in comparison with microbiological diagnosis. * 1 Duplicate answers collected. * 2 Persistent antibiotic-resistant fever>3 days, etc. Cutoff 値 11 pg ml 1 を用いると, 真菌血症に対するβ-D- グルカンの感度は76.1%, 特異度は78.7%, 陽性予測率は 3.1%, 陰性予測率は 99.7% であった 3) 深在性真菌症とβ-D-グルカン値無菌検体の培養検査が提出され, かつβ-D- グルカン測定が同時に行われていた11,902 検体のうち,114 検体で真菌培養が陽性 ( 深在性真菌症 ) であった Cutoff 値を11 pg ml 1 とすると, 深在性真菌症に対するβ-D- グルカンの感度は71.9%, 特異度は68.6%, 陽性予測率は2.2%, 陰性予測率は99.6% であった 4) 抗真菌薬の使用状況 2005 年 8 月から2006 年 7 月の1 年間に抗真菌薬が使用された180 症例 ( 患者数 148 名 ) における抗真菌薬の使用開始理由をTable 2に示す血清診断を根拠としていたのは全体の24.4%( 重複有り ) で, 細菌学的診断を使用開始理由とした場合 (20.6%: 重複有り ) より多かったなお, 血清診断のみを使用開始理由としていたのは25 症例 (13.9%) であった -35-

4 日集中医誌 J Jpn Soc Intensive Care Med Vol. 17 No. 1 Table 3 Colonization index Colonization index n (%) 0/0 35 (23.6) 0/1,0/2,0/3,0/4 86 (58.1) 1/4 2 ( 1.4) 1/3 7 ( 4.7) 1/2,2/4 8 ( 5.4) 2/3 4 ( 2.7) 3/4 1 ( 0.7) 1/1,2/2 5 ( 3.4) Total 148 (100) Colonization index was determined as the ratio of the number of distinct body sites colonized with fungi over the total number of distinct body sites tested. 抗真菌薬使用患者 148 例のうち, 無菌検体から真菌が分離同定されたのは10 例 (6.8%) であったまた, 監視培養指数が0.5 以上だったのは18 例 (12.2%) だが, 監視培養がなされていない, もしくは監視培養指数 0の症例を合わせると121 例 (81.8%) であった (Table 3) 2005 年 4 月から2006 年 3 月の1 年間における各種抗真菌薬別の使用割合ではミカファンギンの使用が最も多く (42.8%), 以下フルコナゾール (36.7%), ミコナゾール (11.7%), ボリコナゾール (7.8%) の順であった同期間の抗真菌薬の薬剤コストは合計で 50,683,512 円であったが, これは同時期に使用されたカルバペネム系薬剤のコスト29,291,536 円と第 4 世代セフェム系薬剤のコスト21,575,162 円を合計したものにほぼ匹敵した考察深在性真菌症診断時, および真菌血症診断時におけるβ-D-グルカンの陰性予測率はそれぞれ99.6%, 99.7% と高かった過去の報告でも指摘されているように,β-D- グルカン測定は偽陽性の多い検査法であるが 9),12), むしろその高い陰性予測率を活用して 3), 深在性真菌症を除外する指標として用いることが適切であるとあらためて結論できるなお, 比濁法による cutoff 値は検査キット説明書の記載どおり11 pg ml 1 に設定することが妥当と考えられた陽性予測率の低さは, 同検査を試行する臨床医がβ-D- グルカン測定を過剰なスクリーニング検査法として用いている可能性も示唆している深在性真菌症の危険因子には, 監視培養での複数陽性や好中球減少, 広域スペクトラム抗菌薬の使用, 長期間にわたるICU 滞在, 外科手術後, 経静脈栄養, 糖尿病, 透析, 重症膵炎などが挙げられる 13) 16) これら危険因子を十分に評価し, 監視培養検査を併用するなどして, 検査前の診断精度を高めたうえで本検査を適用すべきと考える抗真菌薬の使用開始理由において,β-D- グルカン測定をはじめとする血清診断は, 細菌学的診断よりも高い割合で利用されていた抗真菌薬が使用された患者のうち, 無菌検体から培養検査で真菌が同定されたのは6.8% という事実からも, 多くの症例において偽陽性の高いβ-D- グルカン値のみを根拠にしたために, 結果的には無益かもしれない抗真菌薬投与を行っていた可能性が示唆される抗真菌薬が使用された患者の多くは深在性真菌症の危険因子 13) 16) をもつ重症患者であったことから, 治療開始の遅れが生命予後悪化に直接影響することが 2), 多くの臨床医を予防的治療や早期治療介入といった名目での抗真菌薬投与に駆り立てていると思われる前述のように, 抗真菌薬の薬剤コストは広域スペクトラム抗菌薬使用コストとほぼ同じであった真菌血症と確定診断された割合は全血液培養陽性検体の6.3% にすぎないという事実からも, 数少ない真菌感染症に対して治療薬関連の医療資源が過剰に消費されている可能性がある包括支払制度のもとでは考慮すべき問題といえる米国の報告の多くは, 抗真菌薬使用の根拠として監視培養指数を用いている 13) 15),17) Pittetらは, カンジダ感染症の発症の前には同じ遺伝子パターンのカンジダ属の定着が必ず先行することを指摘し, 監視培養指数が0.5 以上であった場合, 後の深在性真菌症の発症リスクが66% となることから, この指数を早期治療の指標として用いることが重要としている 10) 監視培養指数は結果が出るまでに数日を要し,β-D- グルカンは偽陽性が多いため単独での利用には限界があるそこで,Takesue らは2つを組み合わせることで, より簡便で精度の高い補助診断が行えるとしており 17),18), 監視培養指数の普及がより正確な診断と抗真菌薬の適正使用に貢献する可能性があるただし今回の結果からは, 監視培養が行われてない症例が約 4 分の1 存在したため, その診断精度に関しては十分な検討は行えなかった本研究はデータベース解析を主体とした後ろ向き調査であるため, いくつかの欠点を有する β-d- グルカン測定が, あるいはこれを指標とした治療介入の有無が患者の予後に与える影響については, 前向きなコントロールスタディを組むことにより評価する必要があると考えるまた, 剖検により初めて深在性真菌症と確定診断された症例における生前の真菌培養検査所 -36-

Methods: The microbiologic database of a university hospital for the years was analyzed to assess the clinical usefulness of the β-d-glucan assay for the diagnosis of culture-proven fungemia or deepseated mycoses. The reasons for initiation of antifungal therapy were surveyed and the drug costs associated with antifungal therapy were calculated using the pharmacy database for the year Results: 1) Based on the results of receiver operating characteristic curve analysis, the cutoff value of theβ-d-glucan assay for the diagnosis of fungemia was determined to be 11 pg ml 1. 2) The sensitivity and specificity of the β-d-glucan assay using this cutoff value were 76.1% and 78.7%, respectively, and the negative predictive value was 99.7%. 3) Serodiagnosis, including the β-d-glucan assay, was the second most frequent reason (24.4%) for the initiation of antifungal therapy. No microbiologic examination was performed or no fungi were detected in 81.8% of the cases treated with antifungal agents. 4) The drug cost of antifungal therapy was similar to that of broad-spectrum antibiotic therapy. Conclusions: The β-d-glucan assay, with its high negative predictive value, can be used as a screening test to exclude the possibility of deep-seated mycoses. β-d-glucan assay should be prudently conducted and its efficacy should be further investigated taking into consideration the risk factors or colonization index, in order to minimize overtreatment and the associated costs.
