ITP に対するロミプロスチム :romiplostim( ロミプレート ) の効果 (130523) ITP 患者に対する治療法の話題の中でロミプロスチム ( ロミプレート ) についての話題が出たの で その効果を検討してみることにした 学生と一緒に抄読会 ちなみに ロミプロスチムはトロンボポエチン受容体作動薬で 以前勉強したエルトロンボパグと は以下のような違いがあるようだ エルトロンボパグ (eltrombopag) ロミプロスチム (romiplostim) 非ペプチド化合物 ( 低分子化合物 ) ペプチド化合物経口投与皮下注射トロンボポエチン受容体の膜貫通ドメインとトロンボポエチン受容体の細胞膜外ドメイン相互作用に結合 2010 年 6 月現在 EU 加盟 27 カ国を含む 39 米国 EU オーストラリア カナダで ITP にカ国にて ITP に対し承認対し承認 ( レボレード GSK ホームページ http://revolade.jp/practitioner/itp_therapy/thrombopoietin.html) 以前勉強したエルトロンボパグの部分も参照を エルトロンボパグ eltrombopag: レボレードの効果 http://rockymuku.sakura.ne.jp/syounika/erutoronnbopagunokouka.pdf PECO P:234 adult patients with immune thrombocytopenia, who had not undergone splenectomy E:weekly subcutaneous injections of romiplostim (157 patients) C:the standard of care (77 patients) O:Primary end points were incidences of treatment failure and splenectomy. Treatment failure was defined as a platelet count of 20 109 per liter or lower for 4 consecutive weeks at the highest recommended dose, a major bleeding event, or requirement for a change in therapy (including splenectomy) because of an adverse event or bleeding symptoms.
脾臓摘出術を行っていない ITP の成人患者に対して romiplostim を週一回皮下注射すると 標準治療と比較して 治療の失敗や脾臓摘出術が減るかどうかを検討した研究であることが分かる ( 治療の失敗は 4 週連続で観察される 2 万以下の血小板 大出血 副作用や出血症状による治療の変更などが含まれている ) 妥当か 抄録に 52-week study randomly assigned open-label などのキーワードがある ITT 解析については本文の Statistical Analysis に The intention-to-treat patient population, which included all randomized patients, was used in analyses of baseline characteristics and all primary and secondary end points. と記載されている 脾臓摘出術や治療の変更など 医師や患者のバイアスが入りやすい項目が複合エンドポイントの中に含まれているので 本来ならオープンでない方がより良いような気もする どちらかと言うと標準治療群のイベントを増やすようなバイアスがかかる可能性があるので 結果についてはやや割り引いて評価してもいいのかもしれない 結果 治療の失敗や脾臓摘出術は romiplostim 群で 11% 標準治療群で 30% であり romiplostim 群 で少なかった RRR:1-11/30=63.3% ARR:30-11=19% NNT:1/ARR=5.3 6 人 (52 週 ) Patients receiving romiplostim had a significantly lower incidence of treatment failure (18 of 157 patients [11%]) than those receiving the standard of care (23 of 77 patients [30%], P<0.001) (odds ratio with romiplostim, 0.31; 95% CI, 0.15 to 0.61). Splenectomy also was performed less frequently in patients receiving romiplostim (14 of 157 patients [9%]) than in those receiving the standard of care (28 of 77 patients [36%], P<0.001) (odds ratio, 0.17; 95% CI, 0.08 to 0.35). The romiplostim group had a lower rate of bleeding events, fewer blood transfusions, and greater improvements in the quality of life than the standard-of-care group. Serious adverse events occurred in 23% of
patients (35 of 154) receiving romiplostim and 37% of patients (28 of 75) receiving the standard of care. ( 参考文献 1 より引用 ) 標準治療と比べてイベントは大きく抑制していると思うし 重大なイベントが減少する意義は小さ くないと思う 小児に関する論文も読んだ PECO P:children, patients with ITP of 6 months' duration E:subcutaneous injections of romiplostim (n = 17) C:placebo (n = 5) weekly O:increases and maintains platelet counts (with dose adjustments to maintain platelet counts between 50 10(9)/L and 250 10(9)/L.) 6 ヵ月以上持続する ITP の小児に対して romiplostim の皮下注を週 1 回行うと プラセボと比較 して 血小板の維持ができるかどうかを検討した研究であることが分かる 疾患の性格から 血小板を維持できるかどうかも重要な点で 厳密な意味での真のエンドポイン トでは無いかもしれないが 限りなくそれに近いエンドポイントと思う
妥当か 題名 抄録に randomized, double-blind study for 12 weeks, などのキーワードが記載されてい る 少数の試験であり 全例が解析されているようだ ITT 解析については問題なさそう ( 参考文献 2 より引用 ) 結果 血小板を 5 万 /μl 以上にコントロールできたのは romiplostim 群で 88% placebo 群で 0% であ った コントロール不良群が romiplostim 群で 12% placebo 群で 100% とすると RRR ARR NNT は 以下のように計算できる RRR:1-12/100=88% ARR:100-12=88% NNT:1/ARR=1.1 繰り上げると 2 人 (12 週間 ) A platelet count 50 10(9)/L for 2 consecutive weeks was achieved by 15/17 (88%) patients in the romiplostim group and no patients in the placebo group (P =.0008). Platelet counts 50 10(9)/L were maintained for a median of 7 (range, 0-11) weeks in romiplostim patients and 0 (0-0) weeks in placebo patients (P =.0019).
( 参考文献 2 より引用 ) グラフをみても 血小板数に関しての効果は明らか 血小板数を気にしながら学校生活を行う不 自由が少しでも軽くなるなら 治療の意義は大きいような気がしている そもそも 根本的な治療が無い中で 自然治癒を期待しなければならないわけで 小児にとって はステロイド以外の選択枝は多い方がいいと思う 参考文献 1.
