1 Japanese Journal of Endourology(2017)30: CJapanese Society of Endourology 2017 Urologist at Work 高沢亮治北山沙知内田裕將吉田賢辻井俊彦 要旨 経皮的腎砕石術 (PNL) を行う患者を対象に, 腎 盂腎杯と腎結石を実物大に再現した立体モデルを作製して術前シミュレーションを行った. 合計 14 症例の立体モデルを作製し, その有用性を検証した. 全 14 症例において, シミュレーションで予定した第一候補の腎杯へトラクトを作製できた.9 症例は初回 PNLのみでstone-freeを獲得できた. 残りの5 症例は追加治療 (PNL 4 例, 尿管鏡 1 例 ) を要した. 周術期合併症は, 抗菌剤治療の延長を要する38 度以上の発熱が1 例と,48 時間後に発症した心筋梗塞が1 例あった. 輸血を要する出血はなかった. 立体モデルを用いた術前シミュレーションによって, 安定した手術操作を行うことができた. Abstract For patients undergoing percutaneous nephrolithotomy(pnl), we prepared a three-dimensional(3d) model reproducing the renal pelvicaliceal system and kidney stones in full scale and performed a preoperative simulation. We prepared a 3D model in a total of 14 cases and verified its usefulness. In all 14 cases, we could make the tract to the first candidate renal calyx that was scheduled in the simulation. In 9 cases, we achieved a stone-free status with the initial PNL. In the other 5 cases, additional treatment was required(pnl, n=4;ureteroscopy, n=1). Key words: 立体モデル,3Dプリンティング, サンゴ状結石, 腎結石,PNL 緒言 経皮的腎砕石術 (PNL) は, 長径 2cm 以上の腎結石に対する標準的治療である. 近年, 軟性尿管鏡による砕石術の治療成績が向上した一方で, 大きな腎結石に対する経尿道的アプローチの限界もわかってきた 1, 2). 大きな腎結石に対しては,PNLは第一選択として考慮すべき治療である. しかし,PNLに関連する重篤な合併症, すなわち敗血症や輸血を要する出血などが起こる可能性は無視できない 3, 4).PNLの成否を決める重要な操作は, 適切な腎杯にトラクトを挿入する操作である. 大きな結石が腎盂腎杯内を占拠しているため, 適切な腎杯に針を穿刺し, 結石の隙間にガイドワイヤーを通過させ, ダイレータで拡張するという一連の操作は, 通常の腎瘻造設操作に比べて難しい. とくにサンゴ状結石の場合は相当の技術と経験を要する. 一方, 近年 3Dプリンタで作成した骨や腎臓などの実物大の立体モデルを術前シミュレーションへ応用することが注目されている 5, 6). 本邦でもすでに脳神経外科や整形外科, 形成外科領域では,2016 年度診療報酬点数において K939-2 画像等手術支援加算実物大臓器立体モデルによるもの という名称にて2,000 点がついている. そこで, 今回我々はPNLに際して, 術前に腎盂腎杯と腎結石の形状を実物大に再現した立体モデルを作製し, 腎杯穿刺や砕石操作などの術前シミュレーションを行い, その有用性を検討した. 196

(b)extraction of pelvicaliceal system and kidney stone.(c)anterior view of three-dimensional model. 対象と方法 対象は, 当院でPNLを予定した腎結石患者. 本研究に関する患者の同意を得たのち, 造影 CTを0.5mmスライス幅で撮影した. 単純相と排泄相 ( 造影剤注入 7 分後 ) のデータをもとに, 腎盂腎杯と結石の形状を三次元画像解析ソフトSynapse Vincent Version 3 ( 富士フィルム社製 ) を用いて, 三次元構成した. さらに3Dプリンタを用いて立体モデルを作製した. 立体モデルの作製はファソテック社メディカルエンジニアセンター ( 千葉市美浜区 ) に委託し, インクジェット方式 3Dプリンタ (Stratasys 社製 ) を用いて作製した. 試作品による検討後, 腎実質の再現は省略し, 腎盂腎杯の形状を半透明な素材 (TangoPlus FLX930 ) で造形し, その中に赤色の硬質素材 (VeroMagenta ) で色分けして結石を再現した (Fig. 1). 術者は出来上がった立体モデルを参考に, 穿刺する腎杯の選択, 砕石操作の手順, とくにサンゴ状結石の場合は硬性腎盂鏡が到達可能な腎杯の位置などを, 術前に検討してから手術に臨んだ. 全例, 術前に自然尿培養を提出し, 尿培養陽性の場合は, 感受性試験の結果を参考にレボフロキサシンを3-5 日間あるいはセファクロルを3-5 日間, アモキシリンを3-5 日間, 術前経口投与した. 尿培養陰性の場合は, レボフロキサシンを3 日間あるいはセファクロルを5 日間, 術前経口投与した. 周術期の抗菌剤は, セファゾリンを手術当日から 3 日間静脈内点滴投与した. ただし, 術前の自然尿培養でセファゾリンへの感受性がない菌種が判明している症例は, 感受性のある薬剤へ変更した. 全 14 症例中,4 例はアンピシリンを投薬した. 手術は全身麻酔下に修正 Valdivia 体位にておこなった. まず経尿道的に硬性尿管鏡で患側の尿管内を観察したのち, 尿管アクセスシース (Retrace 12/14Fr or 10/12Fr 35cm Coloplast 社 ) を尿管内に挿入した. 軟性尿管鏡 (URF-P6 Olympus 社 ) にて腎盂内の結石と腎盂壁との隙間を観察したのち, 尿管アクセスシースの内筒を使って腎盂内にインジゴカルミン液で青く色づけした造影剤を注入して, 腎盂を拡張した. 超音波およびレントゲン透視ガイド下に経皮的に22G 針で腎杯穿刺をおこない, ダイレータで順次拡張した. 腎盂鏡はMiniperc (Karl Storz 社 ) を使用し,15/18Frもしくは16.5/19.5Frのシースを挿入した. 砕石器はホルミウムヤグレーザ (Versa Pulse Select Lumenis 社 ) もしくはリトクラスト (Lithoclast 2 EMS 社 ) を使用した. 破砕片の摘出は, バスケットカテーテル (Dormia No-Tip 3Fr Coloplast 社 ) を用いた. 手術終了時に,18Fr 腎盂バルーンカテーテルを腎瘻トラクトに, 6Fr 尿管ステントを尿管内に留置した. 術後の発熱と血尿の具合をみて, 順次抜去した. 各症例に関する患者背景 結石情報に加えて, 手術時間, 腎杯を穿刺した回数, 治療結果, 周術期合併症などを集計した. 197

BMI=body mass index, Hb=hemoglobin, Hct=hematocrit, egfr=estimated glomerular filtration rate, CaOx=calcium oxalate, MAP=magnesium ammonium phosphate. 結果 2015 年 9 月から2017 年 2 月までの期間, 合計 14 症例 ( 男性 8 例, 女性 6 例 ) の立体モデルを作製しPNLを行った. Table1に全 14 症例の患者背景および結石情報を示した. 患者年齢は中央値 65.5 歳. 腎盂腎炎の既往は3 例. 術前自然尿培養陽性は10 例 ( 主な菌種 :Streptococcus 3 例,E. coli 2 例,Corynebacterium,Proteus,CNS,gram negative rods,gram positive cocci 各 1 例 ). 術中に採取した腎盂尿培養陽性は2 例, 結石培養陽性は4 例で, いずれも術前自然尿培養の菌種と一致した. サンゴ状結石は6 例. 単純レントゲンKUB 像での結石長径, 外周, 面積 ( 電子カルテ画面上でトレースして測定 ) は, それぞれ中央値 34.5mm,97.5mm,468.5mm 2.CT 像を三次元構成して求めた結石の体積は, 中央値 mm 3 であった. 結石成分分析の結果,60% 以上を主成分と定義すると, シュウ酸カルシウムが11 例, リン酸マグネシウム アンモニウムが 3 例であった. Table 2に手術成績を示した. 手術時間は中央値 133 分. 全 14 症例で, シミュレーションで予定した第一候補の腎杯へトラクトを作製できた. 挿入したトラクト本数は全例 1 本で, 腎杯を穿刺した回数は,1 回が11 例,2 回が2 例,3 回が1 例だった. 手術前後でのHb 値,Ht 値およびeGFR 値の変化は, それぞれ中央値 0.65g/dL, 1.25%,+3.3mL/ min/1.73m 2 だった.PNL 単回でのstone free rateは64%(9 例 /14 例 ) であった.5 例で追加治療を要し, その内訳は4 例で同じトラクトからのセカンドPNL,1 例で尿管鏡を行った. 最終的なstone free rateは86%(12 例 /14 例 ) であった.Stone freeに至らなかった2 症例はいずれも大きな完全サンゴ状結石の症例 (stone volume mm 3 と mm 3 ) で, 同一トラクトから二期的にPNLを行った. 完全なstone freeに至らなかったが, 尿流閉塞解除と腎機能改善という手術目的は達成できた. 周術期合併 198

URS=ureteroscopy. 症は, 術中の腎盂壁の損傷 (Clavien grade I) が1 例, 抗菌剤治療の延長を要する38 度以上の発熱 (Clavien grade II) が1 例, 術後 48 時間以後に心筋梗塞を発症し, 緊急でカテーテルインターベンションを要した症例 (Clavien grade IIIa) が1 例あった. 術後発熱が遷延した症例は, 術前自然尿培養と結石培養でProteusが陽性であった. 心筋梗塞を起こした症例は, 心血管系疾患の既往歴はなく, 術中術後に有意な出血や発熱もなかったため,PNLとの直接的な因果関係は断定できなかった. 輸血を要した症例はなかった. 考察 PNLは長径 2cm 以上の大きな腎結石に対して第一選択とされる標準的治療である.PNLの術中, 術者は術前に記憶したCT 画像と実際に見えるレントゲン透視画像などを頭の中で再構成しながら, 手術操作を行っていると思われる. しかし, これは決して容易な手技ではない. とくに複雑な形状の症例では, 作製したトラクトの位置が悪ければコントロール困難な出血を招いたり, 十分な砕石が行えず手術を終えたりする可能性がある.2011 年のCROES studyの報告によれば, サンゴ状結石に対するPNLの Stone free rateは56.9% にとどまり, 非サンゴ状結石の 82.5% に比べて有意に低く 4), 術後発熱 (14.8% vs 8.7%) や輸血 (9.0% vs 4.5%) といった合併症発生率は有意に高い. したがって,PNLは未だ改良の余地が残る術式である. PNLを成功に導くためには,1 腎盂腎杯に関する正確な解剖学的情報,2 適切な治療計画,3 注意深い手術操作が必要である. 正確な解剖学的情報は, 術前の画像検査に依存するところが大きい. 腎盂腎杯の形状は, 個々の症例ごとに異なる. 一般にPNLの術前にはIVU(intravenous urography) を撮影して, 腎盂腎杯の形状を確認することが推奨されているが,IVUでは平面的な情報しか得られない. 最近の 3D CTU(three-dimensional computed tomographic urography) であれば, 画面上で腎盂腎杯と結石を前後左右に自由に回転させて確認できるので,IVUよりも得られる情報は多い. 今後,3D CTUがIVUの位置づけに取って代わるであろう. しかし, サンゴ状結石のような複雑な症例では,3D CTUの情報でも不十分な場合がある. そこで我々は立体モデルを用いて, さらに正確な解剖学的情報を把握し, 適切な治療計画を立てることによって, 治療成績の向上と修練医への教育の一助となることを期待した. 立体モデルの教育的有用性について異論はないと思われるが, 果たして 199
5 専門医 指導医が行う実際の手術にどの程度貢献するであろうか. 今回, 我々が立体モデルを実際に手に取って感じたのは, 完璧に近い解剖学的情報を把握できたという安心感である. 実物大のモデルを手で触れることによって, 腎盂腎杯と結石の立体構造をイメージしやすくなった. また, トラクトを挿入するのに十分な大きさの腎杯があるか, 結石と腎盂壁との間にガイドワイヤーを通過させる隙間があるか, 挿入したトラクトの位置からどの腎杯まで硬性腎盂鏡が届くかなど, より実践的なシミュレーションを行うことができた. 立体モデルは, 言わば腎盂内の ロードマップ のようであり, とくに複雑な腎結石を治療する際には非常に有用と思われた. 作製したモデルを手術室に持ち込めば, 第 3の情報源としていつでも確認することができる. つまり, 術中は1 視覚的情報 ( 超音波画像 レントゲン透視画像 内視鏡画像 ) と2 触覚的情報 ( 穿刺針や砕石器が結石に当たる感触 ) を,3 立体モデルの情報と照合しながら手術を進めることができる. これまでPNLの術前シミュレーションを目的とした腎盂腎杯の立体モデルに関する報告は少ない.2006 年スウェーデンのRadeckaらは,3Dプリンティングによる腎盂腎杯の立体モデルを初めて報告した 7). 彼らは同時に12 肋骨の位置も再現し, より実践的なトラクト穿刺のシミュレーションを試みている.2014 年英国のTurneyは,3Dプリンティングで製作した腎盂腎杯の立体モデルを使った PNLのトレーニングモデルの有用性を報告している 8) 年ロシアのGadzhievらは, 工作用の油粘土を用いて, 安価で簡便な腎盂腎杯の立体モデルによる術前シミュレーションの有用性について報告している 9). これらの報告のモデルと比較して, 今回我々が作製したモデルの完成度は高い. 腎盂腎杯の形状を半透明な素材で造形し, その中に赤く色分けした結石を再現することによって, より正確な解剖学的情報を有する立体モデルを作成できた. 今回検討した全 14 症例で, シミュレーションで予定した第一候補の腎杯へトラクトを作製できた. ただし,3 例で複数回の腎杯穿刺を要し, いずれもサンゴ状結石の症例であった. うち2 例は第一候補の腎杯へ穿刺したものの, 初回は腎盂尿の逆流を確認できず, 再度穿刺し直してトラクトを作製できた. 残りの1 例は結石容量 mm 3 のとても大きなサンゴ状結石の症例で,3 回の穿刺操作を要した. 第一候補の腎杯に穿刺したが, ガイドワイヤーが十分な位置まで進まなかったため, 第二候補の腎杯を穿刺した. しかし, さらに好ましくない状況であったため, 結局第一候補の腎杯を穿刺し直してトラクトを作製できた. 我々の立体モデルの問題点として, 作製に要する費用と 時間が挙げられる. 今回のモデル作製費用は1 件約 35,000 円で, ファソテック社へのデータ郵送から我々の手元にモデルが届くまでに要する時間は,7-10 日程度であった. 立体モデルによる術前シミュレーションによって, 手術時間の短縮と治療成功率の向上, 合併症率の減少と入院日数の短縮といったプラス効果を期待できるのであれば, トラクト作製が難しいと予想される症例に対しては十分な費用対効果があると考えている. 経験的にトラクト作製が難しい症例は, 完全サンゴ状結石と腎盂が上極側と下極側の2 方向に分岐するdivided pelvisの症例である 10). 完全サンゴ状結石は, 腎杯奥深くまで結石が占拠しているため, 狙った腎杯に穿刺できてもガイドワイヤーをうまく進められないことがある.divided pelvisでは, 腎盂と下極側の分枝を占拠する部分サンゴ状結石が多い印象がある. 水腎症となっている上極側の腎杯を穿刺しても結石全体にアプローチすることは難しい. 一方, 下極側は腎杯の向きが穿刺方向と正対していなかったり, 腎杯が十分拡張していなかったりして, 穿刺が難しいことがある. 立体モデルを用いた術前シミュレーションによって, 術者は腎盂腎杯の解剖学的情報を正確に把握し, 適切な治療計画を立て, それらに裏付けされた安定した手術操作を行うことができた. 今後の課題として, この立体モデルを用いた術前シミュレーションによって, 実際にPNLの治療成績を向上できるのか, 対照群との比較検討をおこないたい. 利益相反自己申告 : 申告すべきものなし 文献 1) Takazawa R, Kitayama S, Tsujii T(2012)Successful outcome of flexible ureteroscopy with holmium laser lithotripsy for renal stones 2 cm or greater. 背部痛などがあげられる 詳細な問診が大切で 臨床症状を確認し 高い確率で病気を診断できる 一方 全く症状を伴わない無症候性血尿では 無症候性顕微鏡的血尿は 放置しても問題のないことが多いが 無症候性肉眼的血尿では 重大な病気である可能性がある 特に 50 歳以上の方の場合は 膀胱がんの可能性があり
