Abstract The patient was a 75-year old woman. Esophagogastroduodenoscopy revealed a submucosal tumor 2cm in size on the anterior wall of the cardiac part of the stomach. Three years later, she visited at our hospital with the chief complaints of tarry stool and dizziness. Proximal gastrectomy was performed, because endoscopically the tumor increased to 7cm in size with ulceration and it was considered to be malignant. Pathological examination was high-risk gastrointestinal stromal tumor. In this case, the submucosal tumor increased by 3.5 times. GIST is needed to be observed carefully and be treated in the appropriate time. Key words : GIST, rapid growth, diagnosis of benign or malignant はじめに従来, 消化管の間葉系腫瘍 (gastrointestinal mesenchymal tumor;gimt) は, 平滑筋由来 ( 平滑筋腫, 平滑筋芽腫, 平滑筋肉腫など ) とされてきた 2).1970 年頃の電子顕微鏡の開発と 1980 年頃の免疫組織化学の発達により, 平滑筋系マーカーにも神経原性マーカーにも染色されない腫瘍群を包括する概念として GIST(gastrointestinal stromal tumor) と総称されるようになった 3) 8). さらに GIST は,1998 年廣田らによって c-kit を Address for Correspondence : Kenji KANAYAMA Department of Medicine and Bioregulatory Science, Graduate School of Medical Sciences, Kyushu University, Maidashi, Higashi-ku, Fukuoka , Japan. kanayama@intmed3.med.kyushu-u.ac.jp 発現するカハールの介在細胞 (interstitial cells of Cajal;ICCs) を起源とする腫瘍であると考えられるようになった 9).GIST に関しては, 現在も良悪性の診断, 予後の予測, それを踏まえての治療方法, 手術適応の時期に関しては確立されておらず, 臨床的に最も重要な問題となっている. 今回われわれは, 約 3 年間で急速な増大経過を示した GIST の1 例を経験したので若干の文献的考察を加え報告する. 症例患者 :75 歳, 女性主訴 : 黒色便, ふらつき感家族歴 : 特記事項なし既往歴 :28 歳時, 子宮後屈.39 歳時, 高血圧.62

3 198 金山兼司ほか 7 名歳時, 狭心症, 胆石症. 現病歴 :1998 年 10 月初診時スクリーング目的にて上部消化管内視鏡検査を施行. 胃噴門部直下前壁に径 2cm 大の正常粘膜で被われた隆起性病変を認めた.1 年後の再検を指示するも受診なく, 2001 年 8 月頃, 黒色便, ふらつきが出現した.10 月再診時 Hb7.1g/dl と高度貧血を認め, 上部消化管内視鏡検査を施行, 胃噴門部直下前壁に径 7 cm 大の大小の潰瘍形成を伴う正常粘膜で被われた隆起性病変を認めた. 新鮮血出血を認めたため止血処置を行い, 加療のため同日入院となった. 入院時現症 : 身長 147cm, 体重 42kg, 眼瞼結膜に貧血を認めた. 入院時検査成績 (Table :Hb 7.1g/dl,Fe 15μ g/dl,uibc 404 μ g/dl,ferritin 3.0ng/dl と鉄欠乏性貧血を認めた. 上部消化管内視鏡検査所見 (1998 年 10 月 ): 胃噴門部直下前壁に径 2cm 大の硬く, 可動性を有する正常粘膜で被われたなだらかな隆起性病変を認めた (Fig.. 超音波内視鏡検査所見 (1998 年 10 月 ): 第 4 層と連続する, 均一な低エコー病変であった (Fig. 2). 上部消化管内視鏡検査所見 (2001 年 10 月 ): 胃噴門部前壁に大小の潰瘍形成を伴う正常粘膜で被われた径 7cm 大の隆起性病変を認めた (Fig. 3). 超音波内視鏡検査所見 (2001 年 10 月 ): 第 4 層と連続する, ほぼ均一な低エコー病変で, 頂部に粘膜欠損を伴い, 径 7cm 大に増大していた. 胃 X 線検査所見 (2001 年 10 月 ): 半立位第 2 斜位像で, 胃穹窿部前壁に長径 8.5cm の頂部に潰瘍を伴う表面平滑な隆起性病変を認めた (Fig. 4). 腹部 CT 所見 (2001 年 10 月 ): 胃穹窿部に, 径 7cm 大の遅延相で増強される腫瘍を認めた (Fig. 5). 手術所見 : 腫瘍の一部が漿膜露出をしていた. 噴門側胃切除術を施行した. 切除標本肉眼所見 : 腫瘍径は cm であった. 中央に潰瘍を有し正常粘膜で覆われた隆起性病変を認めた (Fig. 6). 病理組織学的所見 : 細胞密度は低いが, 紡錘形細胞の束状配列を認めた. また強拡大の 50 視野当たり 5 個以上の核分裂像を認めた (Fig. 7). 免疫組織化学的には,c-kit 陽性 (Fig. 8),CD34 陽性, α-sma 陰性,desmin 陰性,s-100 陰性であった. 以上より GIST と診断され, 悪性度は Fletcher の分類 10) で high risk であった. 術後経過は良好であり, 現在外来通院中である. 考察 GIST は,Rosai により平滑筋細胞と神経細胞への分化の有無によって1 smooth muscle type, 2 neural type,3 combined smooth muscle-neural type,4 uncommitted type の4つのカテゴリーに分類された 1. そして Miettinen らは間葉系腫瘍のうち4を狭義の GIST として取り扱い, c-kit 陽性であることから消化管神経叢周囲に存在し, 蠕動運動を調節する Cajal 介在細胞由来であるとの説を提唱した 12). 免疫組織化学的診断では,c-kit,CD34,desmin,α-SMA,S-100, vimentin 等によって,GIMT の大部分は GIST, 筋原性腫瘍, 神経原性腫瘍にほぼ鑑別される 13). 本症例は免疫組織化学的に,c-kit,CD34 陽性, α-sma,desmin 陰性,S-100 陰性であり,GIST と診断された. EUS-FNA による GIST の組織診断の必要性定腫瘍径については 5cm 以上は cm 未期的な経過観察の重要性それに伴い治療適応お満は 22.4 が悪性といわれていたが最近ではよび治療時期の決定の重要性を再認識させられる 5cm 未満の悪性が 68.6 と増加し腫瘍径だけ症例と考えられたでは良悪性の決定は難しいといわれている急速結増大については市丸ら19) は腫瘍の doubling time が 20 ケ月よりも短いものは悪性の可能性が高いとし大谷ら20)は経過観察中の腫瘍径の変化急速な増大経過を示した GIST の１例を経験したので若干の文献的考察を加え報告したは重要であるとしている潰瘍形成については参考文献潰瘍の有無および表面性状では良悪性の鑑別は困難との報告もある19) 本症例は遠隔転移浸潤は認めなかったが腫瘍径約 7cm であり潰瘍を有していた超音波内視鏡も悪性診断に用いられる腫瘍の辺縁境界については辺縁整は良性辺縁凹凸は悪性とされ内部エコーについては均一は良性不 2) 3) 均一は悪性とされているしかし大きさでは有意差がなく完全な良悪性の鑑別は困難である2 近年超音波内視鏡を用いた穿刺細胞診 4) EUS-FNA が開発され質的診断は向上した今井らは EUS-FNA による正診率を 93 と報告している22) また分子生物学的には c-kit 遺伝子変異が傍細胞膜領域にあるものはそれ以外にある物に比して予後が悪いという報告もあ 5) る23) 本症例は内視鏡上３年前には腫瘍径は約 2cm であり３年後には潰瘍形成も加わり約 7cm に論 6) Stout AP : Tumors of the stomach.
