Data on the perceptions about leadership behaviors performed by nurse managers and motivation for working of 273 staff nurses working at one hospital were collected using questionnaire. The reliability of the scales of leadership behaviors was maintained with Cronbach s alphas above 0.8. The internal consistency of the scales of motivation for working was examined using factor analysis, and the reliability of the scales was measured (Cronbach s alphas ranging from to 0.942). Significant correlation was observed between the perceptions of leadership behaviors and those of motivation for working. Particularly, individualized consideration of transformational leadership had relatively strong positive correlation with all items of motivation for working. 2 Ⅰ. はじめに一般に, 病棟での看護サービスは看護師長をトップとする看護師のチームによって提供されている. リーダーシップは組織の業績に影響を与え, 看護師長のリーダーシップは病棟における看護の質に影響を与えると考えられる. リーダーシップとは, 組織の人間が, 組織目標を達成するように方向づけたり, 動機づけたりする影響力や影響プロセスを指す概念である ( 上田, 2003). 様々なリーダーシップスタイルの中,1980 年代以降は, 複雑で変化が大きい環境下で有効なリーダーシップスタイルとして 変革型リーダーシップ (transformational leadership: 以下 TFL とする ) (Dunham-Taylor, 2000) が注目されている. このようなリーダーシップを発揮するリーダーは, 未来へのビジョンや組織にとっての価値を定めて部下に伝え, そのビジョンのために共に働くことで, 組織のために働く部下の努力を引き出す ( 上田, 2003). TFL には 5 つの構成概念が存在する. 価値や信念, 目的意識を重要視する, リーダーの行動を示す カリスマ性 ( 行動 ), リーダーが自信やパワー, より高い理想や倫理的意識を持っているかどうか, リーダーの特性を示す カリスマ性 ( 特性 ), 創造的に思考し, 問題の解決策を見出せるよう部下に促すリーダーの行動を示す 知的刺激, リーダーが大きな目標を強調してついていきたくなるようなビジョンを示し, そのビジョンが達成可能であることを伝えて部下を奮い立たせる 鼓舞的動機づけ, そして, 部下の一人一人の要求について助言し, 成長と自己実現を促すことで部下の満足感に寄与するリーダーの振る舞いを示す 個別的配慮 である (Antonakis, et al.,2003) 年代以降, 看護領域でも TFL の有効性について議論が始まった (Dunham & Klafehn, 1990) 年には米国で, 全国規模の看護部長やその上司, 部下を対象にした調査が行われ, 看護領域でも TFL が上司や部下の業績評価や, 部下の職務満足 職務意欲と関連することが示唆された (Dunham-Taylor, 2000). 看護師長の TFL に関する研究も行われている. 看護師長の TFL はスタッフナースの職務満足につながる心理的エンパワメントの予測因子となることが明らかとなった (Larrabee, et al., 2003). 本研究は, 看護師長の TFL と, 従来から指摘されている教育指導や対人配慮, 患者配慮, 病棟管理などの行動 ( 吉田ら,1996) についてスタッフナースのとらえ方をもとに調査し, スタッフナースの仕事への意欲との関連を検証することを目的とした. Ⅱ. 研究方法 1. 調査対象および倫理的配慮 A 県内の一般病院 (562 床 ) の全病棟 外来 手術室に勤務する, スタッフナース 273 名のうち, 研究への参加に同意した者を対象に,2004 年 11 月から 12 月にかけて無記名自記式質問紙調査を実施した. 看護師長のリーダーシップに加えてスタッフナースとしての自身の仕事への意欲について質問した. 対象病院の看護部長に調査依頼した際, 本研究の趣旨を書面および口頭で説明し, 研究への理解と同意を得た. 対象者への調査票の配布は病院看護部に依頼した. 記入後の調査票は個別にのり付き封筒に入れて封をした状態で提出するよう依頼し, 個人が特定されないよう配慮した上で各病棟の回収袋に入れ回収した. また, 対象者には, 研究の目的 方法, 個人情報の保護, データ処理の方法, 任意での研究への参加 中止の自由, 研究以外の目的には調査票を使用しないことを書面で説明した. 調査票の回収を持って調査への参加の同意とみなした. なお, 本研究は東京大学大学院医学系研究科 医学部倫理委員会の承認を得て実施した. 2. 使用した尺度 1) リーダーシップ行動に関する項目吉田ら (1996) の 病院における看護婦長のリーダーシップ行動測定尺度の構成 で看護スタッフに対する師長のリーダーシップ行動として集約された行動 49 項目を, 作者の使用許可のもと使用した. この尺度の下位概念は 何かを決定するとき, スタッフの気持ちを配慮する 仕事に対するスタッフの考えを十分に聞く など 10 項目の スタッフ尊重, 何かを決定するとき, スタッフの気持ちを配慮する 仕事に対するスタッフの考えを十分に聞く など 10 項目の 積極的教育指導, 明るく人に接する スタッフに明るく挨拶する など 4 項目の 対 日看管会誌 Vol. 13, No. 2,
3 人配慮, 物品修理等の依頼にすぐに対応してくれる スタッフの待遇改善を積極的に行う など 9 項目の 積極的病棟管理, スタッフの頼みを忘れない 同じ間違いを繰り返さない など 6 項目の 責任遂行, 患者中心の看護をする 患者だけでなくその家族にも目を向けている など 10 項目の 患者理解配慮 の 6 つである. これに加えて,Bass & Avolio(1995) の開発した Multifactor Leadership Questionnaire( 以下,MLQ とする ) の中から, TFL に関連している 20 項目について,MLQ を管理している Mind Garden 社に所定の料金を支払い, 翻訳と研究への使用許可を得た. この部分の下位概念は カリスマ性 ( 行動 ), カリスマ性 ( 特性 ), 知的刺激, 鼓舞的動機づけ, 個別的配慮 の 5 つである. 2) 仕事への意欲に関する項目三隅 (1978) のモチベーターモラール 5 項目と, 林 (1987) のモチベーションなどに関する部分を参考に看護師向けに作成した 今の病棟でこれからも一生懸命働きたい 良い仕事をしたいと思っている など 16 項目, 吉田ら (1996) の看護スタッフの満足度に関する 現在の仕事が楽しい の 1 項目, 加えて 看護の仕事が好きである 患者のために役立ちたいと思う ケアに関する最新の知識 技術を学ぶ などを含む自作の 22 項目の計 44 項目から構成した. いずれの項目も内容に応じて, 5. 非常に~する 4. かなり~する 3. ある程度 ~する 2. あまり~しない 1. ほとんど~しない, あるいは 5. いつも~する 4. かなり~する 3. ときには~する 2. あまり~しない 1. ほとんど~しない といった 5 段階の評定尺度を設けた. 3. 分析リーダーシップ行動については, 既存の尺度を使用しているため信頼性係数 (Cronbach の α 係数, 以下 α 係数とする ) を確認後, 各下位概念の合計点を算出した. 仕事への意欲については, 主因子法, プロマックス回転を用いて因子分析を行った後に α 係数を算出し, 各下位概念の合計点を用いて解析を行うことが妥当であることを確認した. 属性と尺度得点の関連について一元配置分散分析を行った後, 仕事への意欲とリーダーシップ行動の尺度間の相関 を調べた. なお, 尺度の検討や合計得点を用いた解析には下位項目に欠損のない回答のみを用いた. 分析には SPSS version12.0j を用いた. 本研究における有意水準は 0.1% とした. Ⅲ. 結果 1. 回答者の属性調査票はスタッフナース 273 人に配布され,250 人から回収された ( 回収率 91.6%). このうち, リーダーシップと仕事への意欲の各質問項目のうちそれぞれ 3 分の 2 以上回答しているものを有効回答として,244 人を分析対象とした. 回答者の平均年齢は 35.5 歳, 平均看護師経験年数は 13.7 年, 現部署での平均勤務年数は 4.5 年であった. 一元配置分散分析の結果, 属性と尺度得点との間に関連はみられなかった. 2. 尺度選定 1) 看護師長のリーダーシップのとらえ方吉田ら (1996) の看護師長のリーダーシップ評価に関する項目について下位概念の信頼性を検討するためα 係数を算出した結果,α =0.896 ~ であった. また,MLQ の TFL の各質問項目およびα 係数を表 1に示す.TFL の 5 つの下位概念のα 係数は, α =0.846 ~ であった. 2) スタッフナースの仕事への意欲スタッフナースの仕事への意欲に関する 44 項目の質問の回答結果について主因子法によって因子を抽出した後プロマックス回転を施した結果, 表 2のとおり 7 つの因子が抽出された. 各因子に含まれている設問の意味付けを検討した結果,9 項目を除外し, 最終的に 6 つの因子とした. 因子構造の確認のため,9 項目を除外した後, 再び因子分析を行った結果, 想定通り 6 つの因子に収束した. 第 1 因子に高い因子負荷量を示す項目は, 効果的な仕事の仕方について提案する など 8 項目である. よりよい仕事のために積極的に働きかけることを示す項目が中心になっているため, 自己投入( 業務改善 ) とした. 第 2 因子は, 看護の仕事が好きである など 9 項目である. 看護の仕事に愛着を感じている項目が 68 日看管会誌 Vol. 13, No. 2, 2009
4 表 1 変革型リーダーシップの下位概念と項目および Cronbach の α 係数 中心になっているため, 仕事に対する愛着 とした. 第 3 因子は, 患者のために役立ちたいと思う など 7 項目である. 患者のケアに積極的にあたっている項目が中心となっているため, 自己投入 ( 患者ケア ) とした. 第 4 因子は, 今の病棟は自分に合っている などの 4 項目である. 病棟への帰属意識を問う項目であるため, 病棟への帰属意識 とした. 第 5 因子は 仕事を通じて, 大きな満足感を感じる などの 4 項目である. 仕事を通じて自己実現ができているかを問う項目であるため, 仕事による自己実現 とした. 第 6 因子は ケアに関する最新の知識 技術を学ぶ などの 3 項目である. 看護職としての知識面 技術面での成長に関する行動を問う項目であるため, 成長への努力 とした. 次に,6 つの下位概念の信頼性を検討するために, α 係数を算出したところ,α =0.747 ~ であった. 3. リーダーシップ行動得点と仕事への意欲得点の関連リーダーシップ行動得点と仕事への意欲得点それぞれの下位概念同士の関連について表 3に示す. ほとんどの項目同士において 0.1% 水準で有意な相関が見られた. とくに高い相関を示したのは, 個別的配慮 と 病棟への帰属意識 (r=0.404), 知的刺激 と 病棟への帰属意識 (r=0.379), スタッフ尊重 と 病棟への帰属意識 (r=0.360) であった. 特に,TFL の 個別的配慮 は仕事への意欲の下位概念全般と相関が高かった. また, 仕事への意欲の 病棟への帰属意識 はリーダーシップの全ての下位概念と相関が高かった. 日看管会誌 Vol. 13, No. 2,

6 表 3 各下位概念の関連 (Pearson の相関係数 ) Ⅳ. 考察 1. 看護師長のリーダーシップのとらえ方と仕事への意欲の関連 看護婦長のリーダーシップ行動測定尺度 と TFL 両者の信頼性は十分に保たれていたため, これらの尺度を, スタッフナースによる看護師長のリーダーシップのとらえ方を検証するために用いたことは適当であったと考えられる. また, 仕事への意欲に関する質問項目の因子分析では, 意味的に妥当性のあるカテゴリーに因子が収束した. さらに, 命名した 6 つの下位概念の信頼性も十分に保たれていることが示された. よって, 今回使用した質問項目は, スタッフナースの仕事への意欲を問うのに妥当であったと考えられる. 本研究の目的であったリーダーシップのとらえ方と仕事への意欲の関連について, 両者の下位尺度間には関連があることが示された. これはスタッフナースが評価した看護師長の TFL と, 本研究と類似した質問項目を用いて組織へのコミットメントとの関連を示した研究 (McGuire & Kennerly, 2006) と同様の結果であり, 看護師長のリーダーシップ行動を高く評価しているスタッフナースは仕事への意欲も高いことが示唆された. 2. わが国の看護 医療領域における TFL とその可能性近年,TFL の有効性の検討は精神保健領域でも議論され (Aarons, 2006), さらに高齢者ケアに関わるスタッフの心理的 well-being との関連についても明らかにされる (Nielsen, et al., 2008) など, 様々な領域に広がりを見せている. 看護において効果的な TFL を発揮するためには施設による看護師長の評価が行われるべきであり, これはスタッフの職務満足と職場への定着, 患者満足度の向上につながる (Robbins & Davidhizar, 2007). 従って, スタッフの職務意欲向上のために看護師長に TFL のスキルを教育すべきである (McGuire & Kennerly, 2006) といわれている. さらに TFL は看護部長にも求められる資質である (Leach, 2005). わが国では, 看護師長の TFL と看護師の職務満足に関する研究 ( 高谷, 2007) をはじめとして, 看護領域での TFL に関する研究が始まったばかりである. 本研究では従来のリーダーシップ行動と同様に TFL は仕事への意欲と関連があることが示され, わが国においても,TFL がリーダーシップスタイルとして有効である可能性があることが示唆された. 今後, 看護師長および看護部長の TFL に関してさらに研究を進め, 看護師の職務満足や職務意欲に与える影響を明らかにする必要がある. また, 日看管会誌 Vol. 13, No. 2,
7 TFL の特徴の1つである, リーダーと部下が互いを尊重しながら組織の目標に向かっていくこと ( 上田, 2003) は, 病院の目標達成につながるだろう. この点からも, 今後 TFL に関する研究を進めていくことの意義は大きいと思われる. なお, この研究は 1 施設で実施したものであるので, 一般化するためには複数の多様な施設で調査を行い, 得られた結果を検証する必要がある. Ⅴ. 結論 1. 従来提示されていた看護師長のリーダーシップ行動に対するスタッフナースのとらえ方と仕事への意欲の間には関連があった. 2. 看護師長の変革型リーダーシップに対するスタッフナースのとらえ方は, 仕事への意欲と高い関連がみられた. 3. わが国においても看護師長の変革型リーダーシップが有効である可能性が示唆された. 引用文献 Aarons, G.A.
