5 Winter 2014 No. 189 特集 : 中国の社会保障 適度 と 普恵 の視点からみる中国版皆年金体制のゆくえ 1 于洋 要約 2007 年ごろから 中国において 残余主義 福祉から 適度普恵型 福祉への転換論は活発になっている 社会保険制度に関しては 適度 と 普恵 とはともに制度への加入状況 財源政策と給付水準を評価する尺度である 本稿では公的年金制度を例に取り 中国政府が進めている 中国版皆年金 体制に対して 負担と給付の両面から 適度 と 普恵 の尺度を借りて評価してみたい そのうえ 習近平 李克強新政権のもとで進められている 新型都市化 政策と皆年金体制をめぐる一連の課題を考案し これからの中国版皆年金体制のゆくえについて 以下のようなシナリオを推測している 加入者範囲の面から考える 普恵 の目標は到達しており これからは 国民が平等に年金制度による恩恵を受けられるように 制度間の給付調整が行われるだろう 次に 適度 に関しては 基本年金保険における保険料の事業主負担分が引き下げられる方向に向かい 基本年金保険も含めて公務員年金保険 新型農村年金保険と都市住民年金保険の個人負担分も引き上げられる方向になるだろう また 年金給付に関しては 公務員年金の高い給付水準は大幅な引き下げのプロセスに入り 他の年金制度の給付水準も過当にならないように限定させられるだろう それこそ 適度 の真髄であろう キーワード 適度普恵型福祉 公的年金制度 中国版皆年金体制 新型都市化政策 はじめに

18 特集 : 中国の社会保障 中国における 全民低保 の実現 朱珉 要約本稿は 中国の公的扶助の基軸である最低生活保障制度に焦点を当て 1993 年の社会救済制度の改革から 2013 年までの 全民低保 時代までの20 年間を考察する 1990 年代後半に 国有企業改革による大量失業者の生活保障対策として 登場した都市部最低生活保障制度は 1 狭い適用範囲 2 低い保障水準および3 恣意的な運営という3 つの問題を抱えていた 2000 年代に入ると まず都市部で1に対応するため 応保尽保 が 2に対応するため 分類施保 が実施され 制度の整備が行われた 2006 年の 全民低保 大討論を経て 農村部においても制度が成立され 全国の困窮者を包摂する 全民低保 が実現された しかし 量的 質的な制度の拡充は受給者数と給付額の膨張をもたらし いかに給付額を抑えながら すべての困窮者の最低生活を保障するのかという新たな課題が浮上した そのため 中央政府は3に取り組み 制度運用の規範化による地方への介入を強化し 地方政府はワークフェアの導入に乗り出した 以上のプロセスから 中国の公的扶助制度の構築における3つの特徴を指摘する キーワード 最低生活保障 全民低保 ワークフェア 中央政府 Ⅰ はじめに

45 Winter 2014 No. 189 特集 : 中国の社会保障 市場 から 政府 へ 中国における 全民医療保障 政策の成果と課題 李蓮花 要約医療保障の普遍化すなわち 全民医療保障 の実現は 胡錦涛政権期 ( 年 ) の社会保障改革の目玉と言える 農村の医療保障システムの再建から始まった医療改革は SARSの衝撃や医療の不平等に対する国内外の批判を背景に 全民医療保障 へと深化し さらに医療制度全体の見直しにつながった 改革開放以後急速に進んだ医療の市場化 国家責任の後退という傾向に歯止めがかけられ 政府の医療支出も大幅に増加された 制度創設という意味において今回の医療保障改革は画期的なものであったが 一方で 公立病院など提供側の改革はほとんど進まなかったため 医療費が高騰し 患者負担の軽減効果も限定的であった 本稿は 全民医療保障 をキーワードとする 2000 年代の医療保障改革の背景と経緯 その過程で起きた論争を整理したうえで 具体的なデータを通じて今回の医療改革の効果および残された問題を考察し 2020 年の基本的な医療保障の実現に向けた政策の方向性を展望する キーワード 中国 皆保険 市場化 医療格差 政府補助 Ⅰ 研究の背景と目的
